社区康复治疗对“1+1+1”签约脑卒中患者及其家属生存质量和幸福感的影响

摘要目的：研究社区康复治疗对“1+1+1”签约脑卒中患者及其家属生存质量和幸福感的影响。方法：将社区内37名“1+1+1”签约脑卒中患者及35名患者家属纳入研究，37名患者中有男性26例，女性11例，年龄42～89岁，平均年龄（71.08±10.17）岁；35名患者家属中有男性11例，女性24例，年齡24～89岁，平均年龄（59.74±16.75）岁。社区康复治疗团队对患者进行6个月的康复治疗，用世界卫生组织与健康相关生存质量量表（WHO quality of life scale，WHOQOL）和总体幸福感量表（general well-being schedule，GWB）的评分评价社区康复治疗对“1+1+1”签约脑卒中患者及其家属生存质量和幸福感的影响。结果：社区康复治疗后，患者的WHOQOL评分及GWB评分均比治疗前有显著提高（均P<0.05），患者家属的WHOQOL评分及GWB评分均比治疗前也有显著提高（均P<0.05）。结论：社区康复治疗对提高脑卒中患者及其家属的生存质量和幸福感有积极作用。

Effect of community rehabilitation treatment on quality of life and well-being of “1 + 1 + 1”contracted stroke patients and family members

ABSTRACT Objective： To study the effect of community rehabilitation treatment on the quality of life and well-being of stroke patients and their families who signed "1+1+1" contract. Methods： Thirty-seven “1+1+1” contracted stroke patients and 35 patients family members in the community were included in the study， among the 37 patients， there were 26 males and 11 females， and the age was from 42 to 89 years old， with an average age of （71.08±10.17） years old； there were 11 males and 24 females in 35 patients family members， and the age was from 24 to 89 years old， with an average age of （59.74±16.75） years old. The community rehabilitation team performed the rehabilitation treatment for the patient for 6 months， the WHO quality of life scale（WHOQOL） and general well-being schedule（GWB） scores were used to evaluate community rehabilitation treatment for the impact on the quality of life and well-being of the “1+1+1” contracted stroke patients and their family members. Results： After the community rehabilitation treatment， the WHOQOL score and GWB score of the patients were significantly higher than those before treatment（all P<0.05）， the WHOQOL score and GWB score of the patients family members were significantly higher than those before treatment（all P<0.05）. Conclusion： Community rehabilitation treatment has a positive effect on the quality of life and well-being of stroke patients and their family members.

脑卒中是由于脑部血管突然破裂或因血管阻塞导致血液不能正常流入大脑而引起脑组织损伤的一组急性脑血管疾病。脑血管疾病目前仍是全球人类死亡的第二大原因[1]。脑卒中不仅会导致患者躯体残障及语言功能障碍，还会严重影响患者的心理状态[2-4]。脑卒中患者往往难以正确理解和接受自身伤残状态而导致心理自卑感和被歧视感，肢体功能障碍和被歧视感的心理反应使患者出现社交恐惧与焦虑、抑郁等负面情，严重影响脑卒中患者生活技能的恢复，进而也影响患者家属的心理健康状况[5]。本研究旨在探索社区康复治疗对脑卒中患者及其家属生存质量和幸福感的影响。
