【摘 要】目的：探讨加倍剂量厄贝沙坦治疗老年高血压合并慢性肾脏病患者的临床疗效。方法：选择我院于2019年2月至2020年7月期间所收治的78老年高血压合并慢性肾脏病的患者，通过患者的住院号将其分为对照组与实验组，对照组之中应用常规剂量的厄贝沙坦治疗，实验组之中的患者应用大剂量的厄贝沙坦进行治疗，对于患者治疗之后的血压控制状态和尿蛋白变化情况进行统计。结果：通过分析可知，实验组在应用大剂量的厄贝沙坦治疗之后，在血压的控制情况方面明显优于对照组，差异有着统计学的价值（P<0.05）。实验组之中尿蛋白要比对照组下降幅度高，差异较大具有统计学意义（P<0.05）。结论：实施加倍剂量的厄贝沙坦治疗老年高血压合并慢性肾脏病患者，与常规剂量相比较，临床效果更理想，对于患者的血压可以进行更有效的控制，尿蛋白有明显的减少，肾脏也可以在降压的过程中得到保护。

Clinical efficacy of double dose irbesartan in elderly patients with hypertension complicated with chronic kidney disease

【Abstract】Objective： To investigate the clinical efficacy of double dose irbesartan in the treatment of elderly patients with hypertension complicated with chronic kidney disease. Methods：78 elderly patients with hypertension complicated with chronic kidney disease treated in our hospital from February 2019 to July 2020 were selected，accord to their hospitalization number，the patients were divided into control group and experimental group，Routine dose of irbesartan was used in the control group， and large dose of irbesartan was used in the experimental group， The control status of blood pressure and changes of urinary protein after treatment were statistically analyzed. Results： According to the analysis， the experimental group was significantly better than the control group in the control of blood pressure after the application of large dose irbesartan treatment， and the difference had statistical value （P<0.05）. The decrease of urinary protein in the experimental group was higher than that in the control group， and the difference was statistically significant （P<0.05）. Conclusions： Compared with the conventional dose， the clinical effect of double dose irbesartan in the treatment of elderly patients with hypertension complicated with chronic kidney disease is more ideal. The blood pressure of patients can be more effectively controlled， the urinary protein can be significantly reduced， and the kidneys can also be protected during the process of blood pressure reduction.

在心血管疾病之中，高血壓属于一个发病频率比较高的一类疾病，对于人体中的大脑、心脏和肾脏等器官造成影响，老年人因为器官功能发生了退化，高血压合并慢性肾脏疾病术比较常见的[1]。通过应用ARB药物进行降压治疗的同时，可以保护肾脏，其属于对于老年高血压合并慢性肾脏病之中的有效手段。但是仍需要应用大剂量厄贝沙坦进行治疗[2-3]。文章讨论了老年高血压合并慢性肾脏病患者实施大剂量厄贝沙坦治疗的效果。

1 患者在临床试验中依照的相关资料与应用的方法

1.1 基本资料
