摘  要：在信息技術快速发展和互联网全面普及的背景下，我国慢性病治疗策略的转变应结合“体医融合”理念，依托成熟的新媒体技术提高居民健康素养水平，降低慢性病患病率。该文明确了我国慢性病预防当前的主要任务是提高居民健康素养和增强运动锻炼，将我国现阶段慢性病管理模式与医院的防治做对比，指出医院存在一定的局限性且多以治疗为主，设计并采用“互联网+体医融合”模式，运用新媒体技术协助社区医院开展慢性病的预防工作。该小组以南京市迈皋桥社区医院92名高血压、糖尿病患者为目标人群，建立慢性病防治平台，通过推送文章、视频的方式向目标人群普及慢性病健康知识，提供运动指南。基于新媒体的“互联网+体医融合”模式在慢性病管理中，有利于居民了解基本慢性病防治知识，掌握适宜的运;动方法，提高自我管理效能和生活质量，促进自我管理模式的成熟。但实施过程中存在患者积极性不高等问题，仍需根据实际情况选择对应解决策略，进一步完善管理模式。

Design and Application of Chronic Disease Management Model Based on New Media

Abstract： In the context of the rapid development of information technology and the full spread of the Internet， the transformation of our country's chronic disease treatment strategy should be combined with the concept of"physical and medical integration"， relying on mature new media technology to improve residents' health literacy and reduce the prevalence of chronic diseases. This article clarifies that the main task of chronic disease prevention in our country is to improve the health literacy of residents and enhance physical exercise.It compares the current chronic disease management model in our country with the prevention and treatment of hospitals， and points out that hospitals have certain limitations and mostly focus on treatment. We should adopt the"Internet+physical medicine integration"model and use new media technology to assist community hospitals in the prevention of chronic diseases. The group set up a chronic disease prevention and treatment platform with 92 hypertension and diabetes patients from Maigaoqiao Community Hospital in Nanjing City， and popularized chronic disease health knowledge to the target population through articles and videos， and provided exercise guidelines. In the management of chronic diseases， the new media-based"Internet+physical medicine integration"model will help residents understand basic chronic disease prevention and control knowledge， master appropriate exercise methods， improve self-management efficiency and quality of life and promote the maturity of self-management models.However， in the implementation process， there are problems such as low patient enthusiasm， and it is still necessary to choose corresponding solutions according to the actual situation and further improve the management model.
