血清VA、PCT、Hb水平与急性期肺炎患儿病情严重程度的相关性分析

[摘要] 目的 分析血清维生素A(VA)、降钙素原(PCT)、血红蛋白(Hb)水平与急性期肺炎患儿病情严重程度的相关性。 方法 回顾性收集2018年7月至2019年6月在我院就诊的急性期肺炎患儿临床资料83例,根据患儿的病情严重程度分为轻度组(n=33)、中度组(n=29)、重度组(n=21),收集同期于我院体检的正常儿童资料24例为对照组,统计并比较四组的血清VA、PCT、Hb水平,分析上述指标与急性期肺炎患儿病情严重程度的相关性。 结果 四组中,重度组血清VA水平最低,血清PCT水平最高,其次为中度组、轻度组,对照组血清VA水平最高,血清PCT水平最低,差异有统计学意义(P<0.05);四组Hb水平比较,差异无统计学意义(P>0.05);双变量Pearson相关性分析结果显示,急性期肺炎病情严重程度与血清VA水平呈负相关(r=-0.611,P<0.05),与血清PCT水平呈正相关(r=0.863,P<0.05),与Hb水平不相关(r=-0.062,P>0.05);ROC曲线显示,血清VA、PCT预测急性期肺炎病情严重程度的AUC分别为0.869、0.884(均>0.850),有理想的预测价值。 结论 急性期肺炎患儿病情严重程度与血清VA呈负相关,与血清PCT呈正相关,与Hb无相关性,临床可监测血清VA、PCT水平变化,为患儿病情严重程度的早期预测、干预提供依据。

Correlation analysis of serum VA, PCT, Hb level and severity of illness in children with acute pneumonia

[Abstract] Objective To analyze the correlation between serum vitamin A (VA), procalcitonin (PCT), hemoglobin (Hb) levels and the severity of acute pneumonia in children. Methods The clinical data of 83 children with acute pneumonia in our hospital from July 2018 to June 2019 were retrospectively collected. All children were divided into mild group (n=33), moderate group (n=29) and severe group (n=21). Data of 24 children who received normal physical examination in our hospital during the same period was collected as the control group. Serum VA, PCT and Hb levels of the four groups were counted and compared. The correlation between the above indicators and the severity of children with acute pneumonia was analyzed. Results In the four groups, the severe group had the lowest serum VA level and the highest serum PCT level, followed by the moderate group and the mild group. The control group had the highest serum VA level and the lowest serum PCT level. The differences were statistically significant(P<0.05). There was no statistically significant difference in Hb levels between four groups(P>0.05). The bivariate Pearson correlation test analysis showed that serum VA level was negatively correlated with the severity of acute pneumonia (r=-0.611, P<0.05), serum PCT level was positively correlated(r=0.863, P<0.05), Hb level was not correlated (r=-0.062, P>0.05). ROC curve showed that the AUC of serum VA and PCT in predicting the severity of acute pneumonia were 0.869, 0.884(both>0.850), with ideal predictive value. Conclusion Serum VA is negatively correlated with the severity of illness in children with acute pneumonia. Serum PCT is positively correlated with the severity of the illness. Hb has no correlation with the severity of the illness. Clinical monitoring of changes in serum VA and PCT levels can provide evidence for early prediction and intervention of children′s severity.
