[摘要] 目的 探讨降钙素原（PCT）、淀粉样蛋白A（SAA）和C反应蛋白（CRP）在人急性布鲁菌感染中的诊断价值。 方法 选择2014年1月～2016年1月内蒙古医科大学附属医院收治的人急性布鲁菌感染患者100例，根据患者是否发生肝功能损伤（ALI）将患者分为ALI组（n = 39）和非ALI组（n = 61），分析PCT、SAA和CRP在诊断急性布鲁菌感染患者ALI中的临床价值。 结果 与非ALI组比较，ALI组患者谷丙转氨酶、谷草转氨酶、总胆红素、直接胆红素、PCT、SAA和CRP均显著升高，差异均有高度统计学意义（P < 0.01）。PCT、SAA和CRP在诊断疾病布鲁菌感染患者ALI中的曲线下面积分别为0.879、0.858和0.737（P < 0.05）。 结论 PCT、SAA在诊断急性布鲁菌感染患者ALI中具有较好的诊断价值。

[Abstract] Objective To investigate procalcitonin （PCT）， serum amyloid A （SAA） and C reactive protein （CRP） in the diagnosis of acute infection of Brucella. Methods From January 2014 to January 2016， in Affiliated Hospital of Inner Mongolia Medical University， 100 patients with acute brucellosis infection were selected， according to whether patients developed liver function injury （ALI） or not， all patients were divided into an ALI group （n = 39） and a non ALI group （n = 61）. The values of PCT， SAA and CRP in diagnosing ALI in patients with acute infection of Brucella were studied. Results Compared with the non ALI group， levels of glutamic-pyruvic transaminase， glutamic-pyruvic transaminase， total bilirubin， direct bilirubin， PCT， SAA and CRP in ALI group were higher， the differences were statistically significant （P < 0.01）. The area under the curve of PCT， SAA and CRP in the diagnosis of ALI in patients with acute Brucella infection were 0.879， 0.858 and 0.737 （P < 0.05）. Conclusion PCT and SAA have good diagnostic values in the diagnosis of ALI in patients with acute brucellosis infection.

急性布魯菌感染常见于内蒙古和新疆等牧区[1]，严重者可伴游脏器功能损伤，其中以急性肝功能损伤（ALI）较为常见[2-3]。急性布鲁菌感染患者常伴有系统性炎症，不可抑制的炎性反应是导致ALI的主要原因，ALI是患者预后不良的一个主要因素[4-5]。降钙素原（PCT）和淀粉样蛋白A（SAA）近些年来被发现在感染性疾病中具有良好的诊断价值。2016年学者Huang等[6]研究显示PCT在严重脓毒症和脓毒性休克患者中具有较好的诊断价值。2011年Harr等[7]研究显示SAA在诊断创伤患者慢性感染中具有较好的敏感性和特异性。2016年周珣等[8]研究结果显示PCT和C反应蛋白（CRP）在诊断急性布鲁菌病感染中阳性率为67.72%和59.06%。因此笔者推测PCT、SAA和CRP等在诊断急性布鲁菌感染患者ALI中具有一定的临床价值。

1 资料与方法
