【摘要】 本文通过问卷调查、关键知情人访谈、焦点小组讨论等研究方法对上海市宝山区公立医院实施全面预算管理的现状和存在的问题进行了分析，并提出了建立组织架构、制定制度体系、规范操作流程、强化理论培训、加大信息化支撑能力建设等政策建议，为推动公立医院开展全面预算管理，提高公立医院管理水平提供参考和借鉴。

【Abstract】 Through the methods of questionnaire survey， key person interviews and focus group discussion， this study makes an analysis on the status and problems of total budget management in public hospitals of Baoshan district.Then the study puts forward the countermeasures and ideas， such as establishing organization structure， setting up system， standardization of operating procedures， strengthen the ability of information system， so as to provide reference for promoting public hospitals to carry out total budget management.

全面预算管理是一种成熟的、应用广泛的管理工具，能有效调整生产关系并释放巨大的生产力[1-2]。在公立医院开展全面预算管理是社会发展的大趋势，也是当前公立医院改革的要求[3-4]。为了解公立医院实施全面预算管理的现状以及存在的问题，本研究以上海市宝山区为例，对5家区属公立医院开展全面预算管理的整体情况进行了全面调查，并在1家二级甲等综合性医院和1家二级乙等综合性医院内采用分层抽样的方式选取80名职工进行问卷调查，选取包括一线医务人员、中层干部和院级领导在内的30名关键知情人进行访谈。在此基础上，从卫生行政部门和医院管理者两个角度，分析制约公立医院有效落实全面预算管理的原因及其作用机制，明确实施全面预算管理的动力因素和阻力因素，从而制定公立医院落实全面预算管理的总体策略，指导推进公立医院全面预算管理工作。
