早期规范三级康复训练对脑卒中患者功能结局的影响研究

【摘要】目的 探讨早期规范三级康复训练对脑卒中患者功能结局的影响。方法 122例急性脑卒中患者按照随机数字表法分为对照组和康复组，每组各61例。对照组患者接受神经内科常规治疗，康复组患者在对照组基础上接受早期三级康复训练。采用神经功能评定量表（NDS）、Fugl-Meyer运动功能评定量表（FMA）及改良Barthel指数评定量表（MBI）评价治疗效果。结果 入组时，康复组和对照组的MBI、NDS、FMA评分差异均无统计学意义（P＞0.05）。干预6个月后，康复组的NDS评分下降幅度高于对照组，差异具有统计学意义（P＜0.05）；康复组的MBI、FMA评分增加幅度均高于对照组，差异具有统计学意义（P＜0.05）。结论 早期规范的三级康复训练能够有效提升急性脑卒中患者神经功能恢复程度，改善患者的日常生活能力与运动功能。

Effect of Early Three Stage Rehabilitation Training on Functional Outcome in Stroke Patients

【Abstract】Objective To investigate the effect of early rehabilitation on three standardized functional outcome of stroke patients.Methods 122 cases of acute stroke patients were randomly divided into a control group and rehabilitation group，the control group received conventional treatment，rehabilitation group were given three early rehabilitation training based on conventional therapy.Using neurological rating scale，Fugl-Meyer Motor Function Assessment Scale and Modified Barthel Index Rating Scale to evaluate rehabilitation efficacy.Results Beginning of the study，there is no significant difference between the control group and rehabilitation group on the NDS，MBI，FMA.When the intervention of six months，NDS score of rehabilitation group，the difference was statistically significant（P＜0.05）.The rehabilitation group of MBI and FMA scores were significantly higher of increase rate，the difference was statistically significant（P＜0.05）.Conclusion Early tertiary rehabilitation norms of stroke patients can significantly improve the degree of neurological recovery，and significantly improve the activities of daily living and motor function in patients.

脑卒中是急性脑血管循环障碍疾病的总称。随着我国人口老龄化的发展，脑卒中发病率逐年上升。医疗技术的进步与发展使脑卒中患者的存活率逐步提升。伴随着卒中后致残人数也逐年增多。有报道指出约80%的脑卒中存活者存在不同程度的功能障碍［1-2］。因此提高脑卒中患者生存质量，减轻卒中患者运动功能障碍是当前脑卒中患者功能康复的研究热点。有报道指出［3-5］早期规范的康复治疗可以有效的提升脑卒中患者的日常生活能力。并改善卒中患者的功能异常。……
