摘要：目的运用Meta分析方法评价益气补肾、活血通络法治疗糖尿病周围神经病变（Diabetic peripheral neuropathy，DPN）的临床疗效。方法检索中国知网、中国生物医学期刊引文数据库、万方医学网、中国重要会议论文全文数据库、Cochrane Library、MEDLINE、ProQuest、PubMed等数据库中运用益气补肾活血通络法治疗糖尿病周围神经病变的随机对照临床试验文献，严格按照纳入标准筛选合格研究，应用stata120软件进行Meta分析统计相关数据，获得相对可靠的临床治疗证据。结果7篇文献符合纳入标准，共643例患者，Meta分析结果显示：试验组患者的總有效率[OR=153，95%CI（120，195），Z=348，P<001]，正中神经运动神经传导速度（motor nerve conduction velocity，MNCV）[MD=420，95%CI（377，463），P<00001]，腓神经 MNCV[MD=212，95%CI（186，283），P<00001]，腓神经感觉神经传导速度（sensory nerves conduction velocity，SNCV）[MD=281，95%CI（248，314），P<00001]。结论益气补肾、活血通络法治疗糖尿病周围神经病变有一定的临床疗效，但目前研究质量偏低，总体研究例数较少，仍需大数据、多中心的随机双盲临床研究的进一步补充。

在DM的诸多大血管、微血管并发症中，DPN是最常见的，治疗也比较困难[1]。DPN在临床上主要表现为麻木，烧灼，刺痛，顽固性疼痛等异常感觉[2]。DPN会引起神经传导速度的减慢，继而导致保护性感觉的丧失，腱鞘的损害，这些都会增加患者截肢的风险，影响患者的生活质量[3-8]。通常认为DM迁延的病程、体内不稳定的高血糖是导致DPN的主要原因[9，10]。目前对于DPN的研究更加深入，其发病途径主要包括多元醇途径、晚期糖基化终末产物（advanced glycation end products，AGEs）途径、丝裂原活化蛋白激酶途径、Na+/K+-ATPase 通路、多聚ADP核糖聚合酶（PARP）途径、环氧合酶-2（COX-2）途径、炎症信号通路等[11-16]。目前首要治疗是严格控制血糖，西医多采用对症治疗以及对各治病通路的干预作用，以缓解患者的不适，但效果均不佳。DPN在中医学中属消渴伴发痹症、痿症等范畴，当属不通、不荣，消渴总症属阴虚燥热，病久灶着下焦，阴阳两虚，肾为阴阳之本，补肾当为首要；……
