[摘要] 分级诊疗制度在全国推行与实施后，大型公立医院的门诊需求将出现新的变化，普通门诊就诊人次会明显下降，门诊发展将受到影响。新形势下，大型公立医院需进一步明确门诊功能定位，利用“互联网+医疗”发展契机，通过优化门诊设置结构、积极应用信息化技术、创新门诊管理与服务等举措，完成门诊服务重点转型，从而更好地发挥公益性作用，适应并推进分级诊疗。

New thinking on the construction of outpatient service platform in large public hospitals under the background of hierarchical medical system

[Abstract] By implementation of hierarchical medical system， the outpatient demand in major public hospitals will turn and the general out-patient service will obviously decrease， which will have a significant impact on the major public hospitals. Under the new medical reform， in order to better play the public welfare and adapt and promote the hierarchical medical system， major public hospitals need to further clarify the function of outpatient and use internet medical development opportunities to complete the transformation of outpatient service focus， on the basis of optimizing outpatient structure， application of information technology， innovative outpatient management and service.

隨着《国务院办公厅关于推进分级诊疗制度建设的指导意见》（国办发〔2015〕70号）的正式出台，全国各地分级诊疗制度已逐渐建立。这一政策的推行一方面将优化医疗资源的配置和促进基本医疗卫生服务均等化，另一方面也将加剧医疗行业间的竞争。门诊患者的合理分流必然导致大型公立医院普通门诊量的下降，这一形势对大型公立医院的门诊发展提出了新的要求。在分级诊疗不断推进的时候正是“互联网+医疗”迅速发展的时期，“互联网+医疗”的空前发展将带给大型公立医院门诊建设提供更多更新的硬件支持。因此，新时期下，大型公立医院要抓住“互联网+医疗”的发展契机，进一步明确功能定位，创新管理，完成服务重点的转型，才能更好地适应新形势，确保自身长期的差异性的可持续发展[1]。
