摘 要 近年来，随着康复医学的发展，骨科-康复一体化理念越来越受到关注与重视。本文报告在骨科-康复一体化理念指导下，物理治疗对1例老年女性股骨中下段骨折术后患者的康复效果。

Effect of physical therapy of femoral shaft fractures in elderly patients with the concept of integrated orthopedic rehabilitation： a case report

ABSTRACT In recent years， the integrated concept of orthopedic and rehabilitation has attracted more and more attention with the development of rehabilitation medicine. This case reported the rehabilitation effect of physical therapy on an elderly female patient with femoral shaft fracture under the guidance of orthopedics and rehabilitation integration concept.

老年人骨折受骨骼因素和骨外因素的双重影响[1]，女性、高龄、骨质疏松等均是中国老年人骨折的危险因素[2]。一项流行病学研究显示，冬季是老年人骨折的高发季节，多种因素导致老年人骨脆性增加，轻微外伤或者跌倒就易使其发生骨折[3]。另有研究显示，下肢骨折的老年人有很高概率无法恢复其骨折前的行动能力，75%的老年下肢骨折患者即使在骨折愈合后仍然需要依靠助行器辅助行动[4]。而运用骨科-康复一体化理念予以早期介入康复指导和物理治疗，则可让发生下肢骨折的老年患者尽早下地活动，逐步提高其步行能力和生活自理能力，从而使之早日回归家庭和社会[5]。

1 一般资料

患者女，76岁，2019年12月10日在回家途中被电瓶车撞倒在地，无法站立，随后因“右大腿外伤后疼痛、活动受限2 h”入院。查体见右大腿肿胀明显，无淤斑、无张力性水疱，局部皮肤无破裂、无出血，有压痛，无肌肉萎缩，股骨中段有异常活动，有骨擦音、骨擦感，右膝关节活动受限，右侧腓肠肌挤压痛阴性，右下肢肌力5级，右足部感覺正常、活动正常，右足背动脉搏动可触及。……
