苦参碱联用顺铂对HepG2肝癌细胞荷瘤裸鼠的增效减毒作用研究

【摘要】 目的 探討苦参碱联用顺铂对HepG2肝癌细胞荷瘤裸鼠的增效减毒作用。
方法 将HepG2肝癌细胞在裸鼠腋部皮下注射接种,构建出裸鼠皮下移植瘤实验动物模型。将25只瘤径≥5 mm雌性裸鼠(视为造模成功)完全随机划分成5组(每组5只):正常对照组、顺铂组(DDP组,2 mg/kg)、低剂量苦参碱(25 mg/kg)+顺铂(2 mg/kg)组、中剂量苦参碱(50 mg/kg)+顺铂(2 mg/kg)组、高剂量苦参碱(100 mg/kg)+顺铂(2 mg/kg)组。裸鼠成瘤后药物干预14 d,取出瘤体,用千分电子秤称量瘤重,用公式计算抑瘤率。公式计算肾指数、肝指数、脾指数,检测肝、肾功能。
结果 增效方面,各用药组与正常对照组相比瘤重量降低(P<0.05),各剂量苦参碱联用顺铂组与DDP组相比,瘤重量较小(P<0.05)。DDP组及低、中、高剂量苦参碱联用顺铂组抑瘤率依次为58.3%、76.7%、79.6%、82.1%。减毒方面,与DDP组相比,低剂量苦参碱+DDP组、高剂量苦参碱+DDP组血清胆碱酯酶增加(P<0.05)。DDP组较正常对照组BUN升高(P<0.05),低剂量苦参碱+DDP组BUN较DDP组增加(P<0.05),而中、高剂量苦参碱联用顺铂组BUN有回落趋势。低、中、高剂量苦参碱联用顺铂组的肾指数较单用顺铂组高(P<0.05)。
结论 苦参碱与顺铂联用可提高抑瘤率,高剂量苦参碱对顺铂引起的肝损害、肾损害具有一定的缓解作用。

【Abstract】 Objective To investigate the synergism and attenuation effects of matrine (Ma) combined with cisplatin on tumor-bearing nude mice with HepG2 hepatoma cells.
Methods HepG2 hepatoma cells were injected subcutaneously into the axillary region of nude mice to establish an experimental animal model of subcutaneous transplanted tumor in nude mice. A total of 25 female nude mice with tumor diameter ≥ 5mm (regarded as successful modeling) were randomly divided into the following 5 groups (5 mice in each group): normal control group, cisplatin group (DDP group, 2 mg/kg), low dose matrine (25 mg/kg) + cisplatin (2 mg/kg) group, medium dose matrine (50 mg/kg) + cisplatin group (2 mg/kg) group, and high dose matrine (100 mg/kg) + cisplatin (2 mg/kg) group. After 14 days of drug intervention, the tumor bodies were taken out, the tumor weight was weighed by thousandth electronic scale, and the tumor inhibition rate was calculated by formula. In addition, kidney index, liver index and spleen index were calculated by the formula to detect liver and kidney functions.
Results In terms of synergism effect, the tumor mass of each drug group was significantly lower than that of the model group (P < 0.05), and the tumor mass of each dose of matrine combined with cisplatin group was smaller than that of the DDP group (P < 0.05). The tumor inhibition rates of the DDP group and the low, medium and high dose matrine combined with cisplatin groups were 58.3%, 76.7%, 79.6% and 82.1%, respectively. In terms of attenuation effect, compared with the DDP group, serum cholinesterase increased in the low dose matrine + DDP group and the high dose matrine + DDP group (P < 0.05). Compared with the normal control group, the BUN of the DDP group increased (P < 0.05), and the BUN of the low dose matrine + DDP group increased (P < 0.05), while the BUN of the medium dose matrine + DDP group and the high dose matrine + DDP group showed a downward trend. The renal indexes of the low, medium and high matrine + DDP groups were higher than that of the DDP group (P < 0.05).
Conclusion Matrine combined with cisplatin can improve tumor inhibition rate, and high dose of matrine can alleviate liver and kidney damage caused by cisplatin to some extent.

原发性肝癌(简称为肝癌)是全球癌症相关死亡的第三大主要原因,大部分患者在诊断时已属于肝癌晚期,预后差[1]。目前我国肝癌成为第4位(消化系统恶性肿瘤第2位)常见恶性肿瘤及第2位肿瘤致死病因,年龄调整发病率呈逐年下降趋势,但肝癌发病人数仍占全球的55%,肝癌所导致的疾病负担仍呈上升趋势,患者5年生存率无显著提高[2]。肝癌的发生可以将其归因于明确的风险因素(主要是慢性病毒性肝炎和酒精/非酒精性脂肪性肝炎)所导致的肝硬化[3]。目前肝癌的主要治疗手段仍以外科手术切除为主,辅以放化疗和靶向治疗等,近几年肝癌术后的联合化疗成为肝癌治疗的研究热点,其中中药在联合经典化疗药物(如铂类)治疗中的应用特别是其减毒增效作用是肝癌综合治疗的研究焦点,对改善患者生活质量,降低经典化療药物的毒副作用具有不可小觑的意义。……
