[摘要]目的：比较清理术和火针治疗寻常痤疮的疗效和安全性。方法：回顾2018年1月-2019年1月笔者医院皮肤美容中心就诊的40例痤疮患者，根据治疗方案的不同分为火针组和清理术组。火针组：20例，仅火针治疗;清理术组：20例，仅清理治疗。根据治疗前后拍摄的照片及Visia检测结果评估患者治疗前后痤疮的改善程度和治疗后不良反应。结果：两组患者经治疗后痤疮皮损均有不同程度的改善，Visia分析结果显示两组痤疮患者经治疗后其紫质、红色区等各项指标均有改善，火针组效果优于清理术组，且火针组疼痛度明显低于清理术组，具有显著性差异（P<0.05）。治疗导致不同程度的肿胀、红斑及结痂，但这些不良情况在治疗后1～2d内消失，治疗后未发生瘢痕和色素沉着。结论：火针治疗可显著改善痤疮，副作用小、疼痛度轻且费用低廉，值得临床推广应用。

Comparison of the Efficacy and Safety of Cleaning and Fire Acupuncture in the Treatment of Acne  Vulgaris

Abstract：Objective To compare the efficacy and safety of cleaning and fire acupuncture in the treatment of acne. Methods  40 patients with acne treated in the skin cosmetology center of our hospital from January 2018 to January 2019 were divided into fire needle group （n=20） and cleaning group （n=20）.Fire needle group only received fire acupuncture treatment. Cleaning group only received cleaning treatment. According to the photos taken before and after treatment and the results of Visia test， the improvement of acne and postoperative adverse reactions of patients before and after treatment were evaluated.  Results  The acne symptoms of the two groups were improved in varying degrees after treatment. The results of Visia analysis showed that the indexes such as purple substance and red area of the two groups were improved after treatment. The effect of the fire needle group was better than that of the cleaning group， and the pain degree of the fire needle group was significantly lower than that of the cleaning group， with statistical difference （P<0.05）. Treatment resulted in varying degrees of swelling， erythema and scab， but all these adverse conditions disappeared within 1-2 days after treatment. No scar and pigmentation were observed after treatment. The cost of fire needle treatment was significantly lower than that of other treatment schemes.  Conclusion  Fire needle therapy can significantly improve acne with less side effects， less pain and low cost， so it is worthy of clinical application.

寻常痤疮是一种累及毛囊皮脂腺的慢性炎症性疾病，目前在青少年中的患病率超过90%[1]，痤疮有粉刺、丘疹、结节、囊肿等表现，炎症反应重者可遗留瘢痕[2]。其发病机制目前公认的有以下因素：毛囊角质化，皮脂腺分泌增多，痤疮丙酸杆菌及炎症介质释放[3]。中医认为痤疮多由风热之邪客于肺经，阻滞经络致气血瘀滞[4]，或由过食肥甘厚味，致湿热内生，不得下达，上行郁于肌肤而发。痤疮的传统清理术已经被广泛应用于皮肤科，其可以在短时间内将囊肿、粉刺的内容物排出，促进痤疮皮疹的快速愈合。但由于清理治疗是单纯的机械作用而无热作用，对于深度控制不能统一，因而容易造成疼痛、效果不佳及感染的發生，因此火针疗法越来越受到关注。……
