[摘要]目的 探討电项针结合五行音乐疗法辅助治疗脑卒中后抑郁（PSD）的临床效果。方法 选取我院2018年1月～2019年9月收治的60例PSD患者，根据随机数字表法分为对照组与治疗组，各30例，给予对照组患者氟西汀口服治疗，治疗组在对照组的基础上接受电项针结合五行音乐治疗。比较两组患者临床疗效、治疗前后抑郁及生活能力评分。结果 治疗组治疗总有效率高于对照组，差异有统计学意义（P<0.05）;两组治疗后的汉密尔顿抑郁量表（HAMD）评分均低于治疗前，差异有统计学意义（P<0.05），且治疗组的HAMD评分低于对照组，差异有统计学意义（P<0.05）;两组治疗后的日常生活能力（ADL）评分均高于治疗前，差异有统计学意义（P<0.05），且治疗组的ADL评分高于对照组，差异有统计学意义（P<0.05）。结论 电项针结合五行音乐疗法治疗PSD疗效明确，可有效缓解患者抑郁症状。

Clinical effect of electro-nape acupuncture combined with five-element music therapy in the treatment of post-stroke depression

[Abstract] Objective To investigate the clinical effect of electro-nape acupuncture combined with five-element music therapy in the treatment of post-stroke depression （PSD）. Methods Sixty patients with PSD who were admitted to the hospital from January 2018 to September 2019 were selected. They were divided into control group and treatment group by the random table method， each had 30 cases. The control group was treated with oral administration of Fluoxetine， while treatment group was treated with electro-nape acupuncture combined with five-element music therapy on the basis of the treatment of control group. The clinical effect， depression， activities of daily living scores before and after treatment were compared between the two groups. Results The total response rate of treatment group was higher than that of control group （P<0.05）. After treatment， scores of Hamilton depression scale （HAMD） of both groups were lower than those before intervention， the differences were statistically significant（P<0.05）， and HAMD score of treatment group was lower than that of control group， the difference was statistically significant （P<0.05）. The activity of daily living （ADL） scores of both groups after treatment were higher than those before treatment， the differences were statistically significant （P<0.05）， and ADL score of treatment group was higher than that of control group， the difference was statistically significant（P<0.05）. Conclusion Electro-nape acupuncture combined with five-element music therapy is effective in the treatment of PSD， and it can effectively alleviate depressive symptoms of patients.

腦卒中属于急性脑血管疾病，脑卒中后抑郁（PSD）是其常见并发症[1]，发病率达30%以上，临床多表现为低沉、抑郁、焦虑、兴趣减退等，部分患者还会产生轻生念头[2]，对患者身体健康、日常活动造成消极影响。治疗PSD的方法包括药物治疗与非药物治疗，其中药物治疗会对机体造成一定损害，电项针可通过电刺激促使椎动脉血流加快，改善患者机体的血液循环和脑组织的血氧供应，因其安全、有效等优点，被广泛应用于治疗神经类疾病[3]。五行音乐疗法可以调节患者的情绪，通过播放宫、商、角、徵、羽五音，再给予及时有效的心理干预。国内前期已有研究[4]证实，音乐照护可有效改善抑郁症或其他疾病导致的抑郁情绪患者的临床抑郁症状，对其恢复及预后具有积极意义。近几年来，关于两种方法联合治疗PSD的报道并不多见，其应用经验仍有待进一步充实。本研究拟探讨电项针联合五行音乐疗法干预治疗对PSD的效果，以期为后续PSD的临床治疗选择及电项针、五行音乐疗法的推广应用提供依据。……
