【摘要】 目的：观察改良头皮冠状切口在颅颌面骨折手术中的应用价值。方法：将2013年1月-2018年12月在笔者所在医院接受治疗的40例颅颌面骨折手术患者纳入研究对象，遵循随机原则将患者分为观察组（20例）与对照组（20例），分别采用改良头皮冠状切口与传统冠状切口，对两组治疗前后面神经功能评分、生活质量评分及术后并发症发生情况进行综合评价。结果：治疗前，两组面神经功能评分及生活质量评分比较差异无统计学意义（P>0.05），治疗后两组各项评分均优于治疗前，且观察组优于对照组，差异均有统计学意义（P<0.05）。观察组并发症发生率为10.0%，低于对照组的25.0%，差异有统计学意义（P<0.05）。结论：改良头皮冠状切口在颅颌面骨折手术中的应用，安全性高，能够改善患者面神经功能损伤，减少术后并发症的发生，提高患者生活质量，值得推广应用。

[Abstract] Objective： To observe the application value of modified coronal scalp incision in craniomaxillofacial fracture surgery. Method： A total of 40 patients with craniomaxillofacial fracture treated in our hospital from January 2013 to December 2018 were included in the study. They were randomly divided into the observation group （20 cases） and the control group （20 cases）. Modified scalp coronal incision and traditional coronal incision were used respectively. The neurological function score， quality of life score and postoperative complications were evaluated before and after treatment in the two groups. Result： Before treatment， there was no significant difference in facial nerve function score and quality of life score between the two groups （P>0.05）. After treatment， the scores of the two groups were better than those before treatment， and the observation group was superior to the control group， the differences were statistically significant （P<0.05）. The incidence of complications in the observation group was 10.0%， which was lower than that in the control group （25.0%）， the difference was statistically significant （P<0.05）. Conclusion： The application of modified coronal scalp incision in craniomaxillofacial fracture surgery is safe， can improve the facial nerve function injury， reduce the occurrence of postoperative complications， and improve the quality of life of patients. It is worth popularizing and applying.

隨着城市交通及建筑业的不断发展，颅颌面骨折发病率呈现出逐年增高的趋势，伤情复杂，病情严重，严重影响着患者的生活质量[1]。传统临床上针对颅颌面骨折多采用头皮冠状切口治疗，尽管能够对患者病情起到控制作用，然后切口暴露范围大，容易损伤面神经，诱发术区脱发、颞区凹陷等并发症[2]。近年来，临床对传统头皮冠状切口予以改良，经过改良的切口相对隐蔽、瘢痕不明显，有利于颅颌面骨折的修复重建。本研究纳入笔者所在医院收治的40例颅颌面骨折患者，观察改良头皮冠状切口在颅颌面骨折手术中的应用价值，结果如下。

1 资料与方法
