摘 要：目的：探讨高端智能模拟人、情景模拟和PBL（problem based learning）模式相结合在住院医师规范化培训教学中的作用。方法：以SimMan智能模拟人为训练载体，通过构建真实临床情景以PBL的教学模式对新入院的住院医师进行临床思维和基本技能培训。培训后分别进行技能考核和问卷调查。结果：在临床基本技能培训方面，收到了良好的效果，提高了住院医师的临床基础理论、基本技能、病例分析与突发事件的处理能力。结论：应用智能模拟人、情景模拟和PBL模式相结合进行临床技能培训能显著提高培训效果，从而提升住院医师的培训质量。

Abstract： Objective： to explore the function of combining the high-end intelligent simulator， scenario simulation and PBL （problem based learning） in standardizing the residency training. Methods： Taking SimMan intelligent simulation working as an artificial training carrier， the clinical thinking and basic skills of newly hospitalized residents are trained under PBL training mode through constructing a real clinical scenario. After the training， the skills assessment and questionnaire survey are carried out respectively. Results： the effect in training the clinical skills is good. The basic theory and skills， and the ability of case analysis and handling emergencies are improved. Conclusion： the combination of intelligent simulation， scenario simulation and PBL mode can significantly improve the training effect， improving the quality of residency training.

醫疗实践的复杂性决定了其本身是一种对临床技能和综合素质要求很高的行业。现今的临床医师规范化培训教学系统是以书本基础理论为主体，结合临床实习和见习工作进行基础理论联系临床实际从而实现临床培训的目的。由于目前医疗环境日趋严峻，医学生临床培训逐渐受到限制，模拟教学应运而生，可以用尽量贴近真实临床环境和更符合医学伦理的方式进行医学培训教学。模拟教学提倡利用各种手段创造出模拟医疗环境、模拟患者、临床技能训练实验室、模拟手术室、模拟病房乃至模拟医院等软硬件条件，作为临床实习和理论教学的有效辅助手段， 改变旧的教学模式，最大程度提高医学生的临床工作能力和诊疗水平、培养正确而迅捷的临床思维习惯，减少医疗差错和事故在临床工作中的发生。……
