【摘要】目的探讨改良型穿刺法在降低胰岛素泵输液针（PTEE软针）打折率中的应用效果。方法2016年1月至12月期间纳入内分泌科的100例使用胰岛素泵治疗的糖尿病患者作为改善前组，2017年1月至12月期间纳入内分泌科的100例糖尿病患者作为改善后组，改善前组采用捏皮技术进行皮下注射穿刺并留置输液针（PTEE软针）；改善后组采用改良型穿刺法进行皮下注射穿刺并留置输液针（PTEE软针）。对比两组患者胰岛素泵运行期间的输液针（PTEE软针）打折率、堵管率、二次置管率。结果输液针（PTEE软针）打折率、堵管率、二次置管率分别由改善前的11%、2%、13%降低到了改善后的2%、1%、3%，两组患者胰岛素泵输液针（PTEE软针）打折率、二次置管率的比较，差异有统计学意义（P<0.05）。而两组患者胰岛素泵输液针（PTEE软针）堵管率的比较，差异无统计学意义（P>0.05） 。结论改良型穿刺法能降低胰岛素泵输液针（PTEE软针）打折率，值得临床推广应用。

Observation of effect of modified puncture method on reducing

broken rate of insulin pump infusion needle

【Abstract】ObjectiveTo explore the application effect of modified puncture method in reducing the broken rate of insulin pump infusion needle（PTEE soft needle）.Methods100 diabetic patients treated with insulin pump in the Department of Endocrinology from January to December 2016 were included as pre improvement group，and 100 diabetic patients in the Department of Endocrinology from January 2017 to December as post improvement group.The pre improvement group were given skin pinching technology to undergo subcutaneous injection and retained infusion needle（PTEE soft needle）.And the post improvement group were given modified puncture method to undergo subcutaneous injection and retained infusion needle（PTEE soft needle）.And then，broken rate，secondary tube setting rate and tube plugging rate of infusion needle （PTEE soft needle） during the operation of insulin pump in the two groups were compared.ResultsThe broken rate，tube plugging rate and secondary tube setting rate of infusion needle（PTEE soft needle） decreased from 11%，2% and 13% respectively before improvement to 2%，1% and 3% after improvement.The difference of the broken rate and secondary tube setting rate of infusion needle（PTEE soft needle） between the two groups was statistically significant（P<0.05）.But the difference of tube plugging rate between the two groups was not statistically significant（P>0.05）.ConclusionModified puncture method can reduce the broken rate of insulin pump infusion needle（PTEE soft needle），which is worth of clinical popularization and application.

胰岛素泵是一种持续皮下输注胰岛素的治疗系统，能够最大限度模拟人体功能，释放胰岛素，使血糖维持在正常范围内[1～2]。其可在较短的时间内将血糖控制平稳，减少并发症及低血糖的发生。操作方法是将一截长9 mm、输液针头端外径0.52 mm的软管（该软管商品名称为胰岛素泵输液针或PTEE软针）垂直注射并留置在皮下脂肪，通过胰岛素泵装置将胰岛素持续皮下输注。2013年版中国2型糖尿病防治指南建议，在注射胰岛素时应将穿刺部位皮肤适当捏起后再进行穿刺。对于……
