[摘要] 目的 观察补阳还五汤对脑卒中后膝反张中长期的疗效。 方法 将60例经中医辨证属气虚络瘀证的脑卒中后膝反张患者，按照用药时间分成2个月组（A组）、4个月组（B组）、6个月组（C组），每组20例。在均施行神经内科治疗和运动疗法的基础上，按照各组相应的治疗时间予补阳还五汤，各组记录并观察指标至第6个月。疗效观察指标：①膝关节最大伸直度测定；②改良Fugl-Meyer评定（FMA）下肢运动功能。 结果 第4个月及第6个月三组间观察指标有显著差异（P<0.05），第6个月C组的效果优于B组和A组（P<0.05）。 结论 长期应用补阳还五汤不仅能改善脑卒中后膝反张的情况，而且能更好地维持中短期的治疗效果，为指导临床康复提供参考依据。

[Abstract] Objective To observe the long-term effect of Buyang Huanwu decoction on genu recurvatum after stroke. Methods 60 patients with genu recurvatum after stroke who belonged to Qi deficiency and Blood stasis type by Chinese medicine dialectical were divided into two-month group（group A）， 4-month group（group B） and 6-month group（group C） according to the time of treatment， with 20 patients in each group. The patients of the three groups were given Buyang Huanwu decoction according to the corresponding treatment time of each group， based on the implementation of neurological treatment and exercise therapy. The indicators of the three group were recorded and observed to the sixth month. The indicators of the efficacy were observed as follows： ①determination of the knee max straightening angle；②improved Fugl-Meyer assessment（FMA） for lower limb motor function. Results There was significant difference among the three groups at fourth month and sixth month（P<0.05）. The effect of group C was better than that of group B and group A at sixth month（P<0.05）. Conclusion The long-term application of Buyang Huanwu decoction can not only improve the genu recurvatum after stroke， but also can maintain the short and medium term treatment effect well， which provides the reference for guiding clinical rehabilitation.

研究发现约有40%～60%的脑卒中患者在行走时存在膝过伸[1]，其康复效果影响患者后期步行功能的稳定性，也决定着患者的跌倒风险。因此，针对脑卒中患者膝反张的问题进行康复也是目前研究的焦点之一。因其中长期的康复治疗观察鲜有报道，补阳还五汤可增加人体脑组织的局部血流量，抑制血小板聚集，减轻氧化应激损伤。因此，本研究观察补阳还五汤对脑卒中后膝反张中长期的疗效，为指导临床康复提供参考依据。

1 资料与方法

1.1 一般资料

选择2013年6月～2016年6月我院康复科经中医辨证属气虚络瘀证的脑卒中后膝反张患者[2]60例，按治疗时间分成2个月组（A组）、4个月组（B组）、6个月组（C组），每组20例。其中，男42例，女18例，平均年龄（61.2±8.1）岁。脑梗死58例，脑出血2例，右侧偏瘫38例，左侧偏瘫22例，平均病程（14.9±6.0）周。三组在年龄、性别、病程等方面比较无明显差异（P>0.05），具有可比性。……
