中心静脉置管脱管分析及护理体会

【摘要】目的 分析中心静脉置管脱管及护理效果。方法 研究2015年1～12月纳入中心静脉置管脱管共52例，分析脱管原因，并随机分两组。26例施行常规护理，入对照组；26例施行综合护理，入研究组。组间比较。结果 两组中研究组的置管时间更长，满意度更高，差异有统计学意义（P＜0．05）。结论 中心静脉置管脱管采用综合护理效果佳。

Objective Make an analysis on removal of central venous catheter and its nursing efficacy． Methods Chose 52 cases of central venous catheter removal happened in hospital from January 2015 to December 2015，analyzed the cause for removal and then separated them into two groups，26 cases in control group were given conventional nursing，while another 26 patients in study group were given comprehensive nursing，and then compared nursing effects between two groups． Results Central venous catheter was kept unmoved for a longer time and patients’ satisfaction rate was much higher in study group compared to counterparts in control group，there was a differential between two groups and such a differential had statistic value（P＜0．05）．Conclusion Comprehensive nursing is much more effective for central venous catheter removal．

中心静脉置管为各种营养及药物输注重要方法，应用较广。脱管为导致非计划拔管主要原因，患者未经临床同意自行拔除导管，管意外脱落，或者操作不当引起拔管。发生脱管后，直接影响正常治疗，并对患者有一定损伤，使住院日延长，费用增加［1］。此文研究2015年1～12月纳入中心静脉置管脱管共52例，分析脱管原因，总结综合护理方法与实施效果，具体见下。

1 资料和方法

1.1 一般资料

研究2015年1～12月纳入中心静脉置管脱管共52例，分析脱管原因，并随机分两组。26例施行常规护理，入对照组，单腔10例，16例双腔，年龄13～79岁，平均（40.15±3.27）岁，女患者14例，12例男患者。26例施行综合护理，入研究组，单腔11例，15例双腔，年龄12～80岁，平均（40.21±3.21）岁，女患者13例，男患者13例。组间差异不具有统计学意义（P ＞0.05）。

1.2 方法

1.2.1 置管宣教 置管前，需加强宣教，按个体病情在合适部位穿刺，并避免凹凸不平、出汗多及活动量大部位，减少固定难度。说明置管期间应注意的事项，使其积极配合，减少脱管。

1.2.2 导管固定 给予外固定和缝合，延长置管时间，减轻痛苦。……
