[摘要] 目的 對穴位按摩联合穴位贴敷减轻初次腹膜透析置管术后疼痛的有效性进行观察分析。 方法 选取2018年4月至2019年9月我院肾内科腹膜透析置管术后患者80例，随机分为对照组和研究组，每组各40例。对照组采取常规术后指导，研究组采取常规术后指导加穴位按摩联合穴位贴敷。参照疼痛视觉模拟评分法（VAS），对两组患者术后疼痛减轻情况、舒适度、护理满意度进行比较分析。 结果 两组患者术后VAS评分随时间增加均逐渐下降，研究组术后12 h、术后1 d、术后2 d VAS分值均明显低于对照组，差异有统计学意义（P<0.05）。研究组患者舒适度高于对照组，差异有统计学意义（P<0.05）。两组护理满意度比较，差异无统计学意义（P>0.05）。 结论 初次腹膜透析置管术后患者应用穴位按摩联合穴位贴敷可减轻其疼痛，效果好，可推行。

Observation on the application of acupoint massage combined with acupoint application in relieving pain of patients after primary peritoneal dialysis catheterization

[Abstract] Objective To observe and analyze the efficacy of acupoint massage combined with acupoint application in relieving pain of patients after primary peritoneal dialysis catheterization. Methods From April 2018 to September 2019， a total of 80 patients with peritoneal dialysis after catheterization admitted to the department of nephrology of our hospital were randomly divided into the control group （n=40） and the study group （n=40）. The control group was given routine postoperative guidance， while the study group was given routine postoperative guidance and acupoint massage combined with acupoint application. According to the pain visual analogue scale（VAS）， the postoperative pain relief， comfort level and nursing satisfaction degree of the two groups were compared and analyzed. Results The VAS scores of the two groups decreased gradually with the increase of time， and the VAS scores at 12 hours， 1 day， 2 days after surgery of the study group were statistically significantly lower than those of the control group（P<0.05）. The comfort level of patients in the study group was higher than that in the control group， and the difference was statistically significant（P<0.05）. There was no statistically significant difference in nursing satisfaction degree between the two groups（P>0.05）. Conclusion Acupoint massage combined with acupoint application relieve the pain of patients after primary peritoneal dialysis catheterization， which has good efficacy and should be implemented.

腹膜透析置管术是终末期肾病患者进行腹膜透析采用的手术方法，腹部手术为一种创伤性手术，造成腹部组织机械性损伤，导致某些化学物质（如缓激肽、蛋白水解酶、5-羟色胺等）释放增加，刺激神经末梢引起疼痛，对机体造成的组织损伤不仅包括皮肤，还会累及到肌肉组织、器官等，组织受到手术伤害，让患者感到疼痛不适[1];透析置管术后产生的胀痛，导管出口处的钝痛等对患者心理及术后恢复产生极大的影响，疼痛已成为第5大生命体征[2]，因此采取及时有效的措施来缓解患者疼痛具有重要的临床意义。本研究主要对穴位按摩联合穴位贴敷减轻初次腹膜透析置管术后患者疼痛的效果进行总结，现报道如下。……
