摘 要 目的：回顾临床药师对抗肿瘤药物应用的点评及干预，为临床合理使用抗肿瘤药物提供参考。方法：回顾临床药师2018年间参与的复旦大学附属华山医院北院抗肿瘤药物全医嘱审核结果，分析不合理用药情况。结果：抗肿瘤药物不合理用药医嘱共169份，存在的不合理问题主要为溶剂选择不合理、溶剂用量不合理、给药剂量不合理、给药顺序不合理和给药途径不合理等。经与病区医师沟通，对每月不合理医嘱进行干预，不合理医嘱占比呈随季度下降趋势。结论：临床药师的全医嘱审核能保障临床合理用药，促进抗肿瘤药物的规范使用。

Analysis of the effectiveness of clinical pharmacists participating in the review of antitumor drugs*

ABSTRACT Objective： To promote rational drug use in the clinic by investigating the current status of the use of antitumor drugs and rectifying irrational medications immediately in the ward by clinical pharmacist. Methods： The use of antitumor drugs in northern division of Huashan Hospital affiliated to Fudan University during 2018 was retrospectively reviewed and analyzed by clinical pharmacists. Results： There were a total of 169 medical orders for irrational use of antitumor drugs. The irrational problems were mainly included in the choice and dosage of solvent， the dosage of drugs and the order and route of administration. The proportion of irrational medications was quarterly declined by communicating with ward doctors and intervening on monthly irrational doctor orders. Conclusion： The full review of doctors orders by clinical pharmacists can ensure the rational use of clinical drugs and promote the standardized use of antitumor drugs.

惡性肿瘤发病率高，且其死亡率仅次于心、脑血管疾病，严重威胁人们的生命和健康。化疗仍是目前肿瘤治疗最重要的手段，但由于抗肿瘤药物均存在一定的毒性，所以合理使用抗肿瘤药物非常重要，与肿瘤治疗效果和药物不良反应直接相关。临床药师在促进抗肿瘤药物规范使用方面起着关键作用。

复旦大学附属华山医院北院（以下简称为“我院”）是上海市政府“5+3+1”医疗服务工程重点项目，是一家集医、教、研于一体的三级甲等综合性医院。……
