摘要 目的：探討星状神经节阻滞联合头痛宁胶囊治疗慢性紧张性头痛合并失眠的临床疗效。方法：选取2018年2月至2018年12月北京市房山区第一医院疼痛科收治的慢性紧张性头痛合并失眠患者80例作为研究对象，按照随机数字表法分为对照组和观察组，每组40例。对照组单纯实施星状神经节阻滞，观察组联合口服头痛宁胶囊。采用视觉模拟评分法（VAS）和睡眠质量（QS）评分对2组治疗前、后进行疼痛程度和睡眠评分，评估临床治疗效果，同时观察不良反应。结果：对照组和观察组治疗后VAS评分和QS评分均较治疗前降低，差异有统计学意义（P<0.05），观察组VAS和QS评分明显低于对照组，差异有统计学意义（P<0.05）。2组均无严重不良反应。结论：星状神经节阻滞联合头痛宁胶囊可有效缓解头痛，改善睡眠，联合治疗疗效优于单一治疗，且未见明显不良反应。

Clinical Observation of Stellate Ganglion Block Combined with Headache Capsule in the Treatment of Chronic Tension Headache Combined with Insomnia

Abstract Objective：To explore the clinical effect of stellate ganglion block combined with touningcapsule in the treatment of chronic tension headache with insomnia.Methods：A total of 80 patients with chronic tension headache and insomnia who were admitted to the pain department of our hospital were enrolled and randomly divided into the control group and the observation group，with 40 cases in each group.In the control group，stellate ganglion block was performed alone，while the observation group was combined with oral headache capsule.The visual analogue scale（VAS）and the quality of sleep（QS）scores were used to evaluate the pain degree and sleep score before and after treatment.At the same time，adverse reactions were observed.Results：The VAS score and QS score of the control group and the observation group were lower than those before treatment（P<0.05）.The VAS and QS scores of the observation group were significantly lower than those of the control group（P<0.05）.There were no serious adverse reactions in either group.Conclusion：Stellate ganglion block combined with headache capsule can effectively relieve headache and improve sleep.The combined treatment is better than single treatment，and no obvious adverse reactions are observed.

慢性紧张性头痛是是临床上常见的慢性头痛，约占原发性头痛的40%，随着科技水平的进步，人们的社会压力不断增加，生活方式大大改变，头痛发病率逐年上升，紧张性头痛（Tension-Type Headache，TTH）患者门诊就诊率也在增加。患者主要表现为双侧枕位、额颞位或全头压痛，呈胀痛或发紧样痛，可伴有恶心呕吐、畏光、失眠等症状。就诊患者多数因长期慢性疼痛合并了失眠，专家研究还发现长期疼痛会导致睡眠障碍，紧张性头痛的发生率随着长期失眠也会增加，日常生命质量严重下降[1-2]。目前抗抑郁药是治疗TTH的首选，但长期服用可能发生认知功能损害、药物依赖等不良反应[3]，目前我们急需寻找安全有效的治疗方法[4]。星状神经节阻滞广泛应用于治疗头、面、颈、胸、上肢甚至全身的疼痛和非疼痛性疾病[5]，近些年报道用于治疗失眠症疗效显著[6]。……
