【摘要】 目的：研究中药塌渍TDP照射减轻患者下肢闭合性骨折早期肿胀的护理效果。方法：选取2019年3-11月笔者所在医院下肢闭合性骨折肿胀患者138例，按照入院顺序将其随机分为观察组和对照组，各69例。对照组采用TDP治疗，观察组采用中药塌渍TDP治疗。比较两组患肢周径、术前等待时间、住院时间及满意度。结果：观察组入院后72 h患肢周径小于对照组，差异有统计学意义（P<0.05）。观察组术前等待时间、住院时间均明显短于对照组，差异有统计学意义（P<0.05）。观察组满意度明显高于对照组，差异有统计学意义（P<0.05）。结论：中药塌渍TDP照射用于减轻下肢闭合性骨折早期肿胀效果明显，可缩短等待手术时间，是一种简便安全有效的传统医学治疗方法。

[Abstract] Objective： To observe the nursing effect of traditional Chinese medicine collapse TDP irradiation on reducing early swelling of lower limb closed fractures. Method： A total of 138 patients with lower limb closed fractures and swelling in our hospital from March to November 2019 were selected and randomly divided into the observation group and the control group according to the order of admission， with 69 patients in each group. The control group was treated with TDP therapy， the observation group was treated with traditional Chinese medicine collapse TDP therapy. The affected limb circumference， preoperative waiting time， hospitalization time and satisfaction were compared between the two groups. Result： The circumference of the affected limb 72 h after admission in the observation group was less than that in the control group， and the difference was statistically significant （P<0.05）. The preoperative waiting time and hospitalization time in the observation group were significantly shorter than those in the control group， and the differences were statistically significant （P<0.05）. The satisfaction of the observation group was significantly higher than that of the control group， and the difference was statistically significant （P<0.05）. Conclusion： Traditional Chinese medicine collapse TDP irradiation is effective in reducing early swelling of lower limb closed fracture， shortening preoperative waiting time， and is a safe， convenient and effective method of traditional Chinese medicine treatment.

下肢閉合性骨折因损伤周围组织，早期肿胀疼痛明显，不适宜立即手术，因此术前应提供优质治疗护理管理方法迅速消肿，尽早手术已成为中医临床研究的突破点。《外科精义》云“溻渍疮肿之法，宜通行表，发散邪气，使疮内消也”，说明溻渍法是经肌肤腠理贯络通经而发挥治疗作用[1]。笔者应用中药塌渍TDP治疗对下肢闭合性骨折消肿镇痛效果良好，现总结如下。

1 资料与方法

1.1 一般资料

选取2019年3-11月笔者所在医院下肢闭合性骨折肿胀患者138例，纳入标准：骨折无开放性伤口且受伤12 h内入院。排除标准：基础疾病影响手术时间者;其他原因不能手术者;依从性差者。入院后由责任护士测量患者肢体周径并记录。肿胀值=患侧肢体周径-健侧肢体周径，依据《中药新药临床研究指导原则》，轻度肿胀：肿胀值<0.5 ㎝，较正常皮肤肿胀，但皮纹还在;中度肿胀：肿胀值0.5～1 ㎝，较正常皮肤肿胀，但皮纹消失;重度肿胀：肿胀值>1 ㎝，出现张力性水泡[2]。按照入院顺序将其随机分为观察组和对照组，各69例。观察组男39例，女30例;年龄20～40岁20例，41～60岁35例，61～70岁14例，平均年龄（46.65±14.17）岁;股骨骨折14例，髌骨骨折13例，胫腓骨骨折20例，踝骨折13例，跟骨骨折9例;肢体轻度肿胀25例，中度肿胀30例，重度肿胀14例。……
