【摘要】 目的：探讨全髋关节置换术和人工股骨头置换术治疗老年股骨颈骨折患者的手术效果。方法：研究共选择88例老年股骨颈骨折患者作为资料，均为本院2016年2月-2019年2月收治确诊，随机分为对照组和观察组，各44例，两组患者分别行人工股骨头置换术、全髋关节置换术。评价两组髋关节功能恢复效果及并发症率。结果：观察组髋关节功能恢复优良率为86.36%，对照组为88.64%，组间比较差异无统计学意义（P>0.05）;观察组并发症发生率6.82%，对照组发生率9.09%，组间比较差异无统计学意义（P>0.05）。结论：针对老年股骨颈骨折患者行全髋关节置换术或人工股骨头置换术，均获得良好的治疗效果，临床选择手术方式时，需充分考虑患者年龄、基础疾病、身体状况、手术麻醉风险及日常生活需求，方能取得临床最佳效果。

Effect of Total Hip Replacement and Artificial Femoral Head Replacement on Fracture of Femoral Neck in Elderly Patients/XIE Baiyu. //Chinese and Foreign Medical Research， 2020， 18（19）： -125

[Abstract] Objective： To investigate the effect of total hip arthroplasty and artificial femoral head arthroplasty in elderly patients with femoral neck fractures. Method： A total of 88 elderly patients with femoral neck fractures were selected as the data， all of whom were admitted and diagnosed in our hospital from February 2016 to February 2019. The patients were randomly assigned to the control group and the observation group， with 44 cases in each group， and the two groups underwent artificial femoral head replacement and total hip replacement， respectively. Result： The rate of hip joint function recovery was 86.36% in the observation group and 88.64% in the control group， with no significant difference （P>0.05）. The incidence of complications was 6.82% in the observation group and 9.09% in the control group， with no statistically significant difference （P>0.05）. Conclusion： Total hip arthroplasty or artificial femoral head replacement for elderly patients with femoral neck fractures has achieved good therapeutic effect. The best clinical effect can only be achieved if the patients age， basic diseases， physical condition， risk of surgical anesthesia and daily life needs are fully considered when choosing surgical methods.

虽然如今临床治疗股骨颈骨折方法较多，尤其尽早手术已形成共识，治疗效果好，但老年患者具有耐受力低、合并多种慢性疾病等特征，手术风险较高，恢复慢，因此需根据患者年龄、身体状况、生活能力需求、基础疾病情况决定手术妨害[1]。全髋关节置换术和人工股骨头置换术在老年股骨颈骨折治疗中备受重视，可恢复髋关节功能，而不同学者对两种术式的理解存在差异，具体选择何种方案成为临床研究重点[2]。为此，本次研究对全髋关节置换术和人工股骨头置换术治疗老年股骨颈骨折患者的临床效果，具体报告如下。
