【摘要】 目的：分析高速涡轮牙钻联合牙挺法治疗下颌近中阻生齿的效果及安全性。方法：选择2017年9月-2019年2月筆者所在医院收治的85例下颌近中阻生齿患者，按随机数字表法将其分为对照组（42例）和研究组（43例），对照组接受常规拔牙器械拔除患牙，研究组采用高速涡轮牙钻联合牙挺法治疗。比较两组临床指标、并发症发生情况及满意度。结果：研究组手术时间、肿胀程度、张口受限程度和牙槽窝完整性分别为（13.10±2.23）min、（14.91±0.99）mm、（3.49±1.03）mm、（1.83±0.25）分，均优于对照组，差异有统计学意义（P<0.05）。研究组并发症发生率为4.65%，明显低于对照组的19.05%，差异有统计学意义（P<0.05）。研究组满意度为97.67%，高于对照组的80.95%，差异有统计学意义（P<0.05）。结论：高速涡轮牙钻联合牙挺法治疗下颌近中阻生齿，能减轻患者术后肿胀程度和张口受限程度，防止损伤牙槽窝完整性，术后并发症少，患者治疗满意度高，值得临床推广和使用。

【Abstract】 Objective：To analyze the effect and safety of high-speed turbine dental drill combined with dental malleolus in the treatment of mandibular proximal and middle impacted teeth.Method：A total of 85 patients with mandibular proximal and middle impacted teeth admitted in our hospital from September 2017 to February 2019 were selected.They were divided into the control group（42 cases） and the study group（43 cases） according to the random number table method.The control group received conventional tooth extraction instruments，and the study group was treated with high-speed turbine dental drill combined with dental malleolus.The clinical indicators，complications and satisfaction of the two groups were compared.Result：The operation time，swelling degree，mouth opening restriction degree and alveolar socket integrity of the study group were （13.10±2.23）min，（14.91±0.99）mm，（3.49±1.03）mm，and （1.83±0.25）points，respectively，which were all better than those of the control group，and the differences were statistically significant（P<0.05）.The complication rate of the study group was 4.65%，which was significantly lower than 19.05% of the control group，the difference was statistically significant（P<0.05）.The satisfaction of the study group was 97.67%，which was higher than 80.95% of the control group，the difference was statistically significant（P<0.05）.Conclusion：The high-speed turbine dental drill combined with dental malleolus for the treatment of mandibular proximal and middle impacted teeth can reduce the degree of postoperative swelling and mouth opening restriction degree，prevent damage to the integrity of alveolar socket，and reduce postoperative complications，and high satisfaction with the treatment of patients.It is worthy of clinical promotion and use.

下颌近中阻生齿在口腔科较为常见，若患者未能接受有效治疗，通常会因病情迁延难愈易引发冠周炎、邻牙龋齿或牙齿间隙感染等一系列并发症，在一定程度上会对患者日常生活带来严重影响。此外，下颌近中阻生齿位置特殊，易出现牙龈撕裂或邻牙损伤[1-2]。因此，患者一旦出现下颌近中阻生齿，需及时接受有效治疗。本研究针对笔者所在医院2017年9月-2019年2月收治的85例下颌近中阻生齿患者采取不同治疗方案，对其效果进行分析，现报告如下。

1 资料与方法

1.1 一般资料

选择2017年9月-2019年2月笔者所在医院收治的85例下頜近中阻生齿患者临床资料，纳入标准：下颌近中阻生齿确诊者，资料完整者。按随机数字表法将其分为两组。对照组42例，男22例，女20例;年龄21～49岁，平均（35.06±11.72）岁;高中19例，大专以上23例。研究组43例，男21例，女22例;年龄23～52岁，平均（34.89±11.95）岁;高中20例，大专以上23例。……
