血清HMGB1水平与慢性阻塞性肺疾病关系

[摘要] 目的 探討慢性阻塞性肺疾病患者血清高迁移率族蛋白B1(high mobility group protein B1,HMGB1)水平与慢性阻塞性肺疾病的相关性。 方法 选取我院2015年1月~2016年5月收治的慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者共计73例,为COPD组。另选取73例健康研究对象为对照组。通过用力呼气量占用力肺活量比值(FEV1/FVC%)、用力呼气量(FEV1)占预计值%评价两组研究对象的肺功能;采用酶联免疫吸附试验(ELISA)方法检测两组研究对象血清HMGB1水平并进行比较,分析血清HMGB1水平与肺功能的相关性。 结果 对照组FEV1/FVC%、FEV1占预计值%显著高于COPD患者组,差异有统计学意义(P<0.01);对照组血清HMGB1水平均值为1.75 μg/L,COPD患者组为3.84 μg/L,COPD患者血清HMGB1水平显著高于对照组,两组数值比较,差异有高度统计学意义(P<0.01);COPD患者血清HMGB1水平与FEV1/FVC%和FEV1占预计值%均呈负相关关系,差异有高度统计学意义(P<0.01)。 结论 COPD患者血清HMGB1水平显著升高,且与肺功能呈负相关,提示血清HMGB1水平与慢性阻塞性肺疾病严重程度相关,对于疾病严重程度的判断具有一定的提示意义。

[Abstract] Objective To investigate the correlation between serum high mobility group protein B1(HMGB1) and chronic obstructive pulmonary disease(COPD) in patients with COPD. Methods A total of 73 patients with COPD who were treated in our hospital from January 2015 to May 2016 were selected as COPD group. Another 73 healthy subjects were selected as the control group.Forced expiratory volume in 1 second/forced vital capacity capacity(FEV1/FVC%) and percentage of predicted forced expiratory volume in 1 Second(FEV1) were used to evaluate the lung function of the two groups. The enzyme-linked immunosorbent assay(ELISA) was used to detect the serum HMGB1 levels of the two groups and the serum HMGB1 levels between the two groups were compared. The correlation between serum HMGB1 levels and pulmonary function was analyzed. Results The levels of FEV1/FVC% and FEV1% in the control group were significantly higher than those in the COPD group ,and the difference was statistically significant(P<0.01). The serum HMGB1 level was 1.75 μg/L in the control group and 3.84 μg/L in the COPD group The serum HMGB1 level in patients with COPD was significantly higher than that in the control group,and the difference was statistically significant(P<0.01). There was a negative correlation between the levels of serum HMGB1 and FEV1/FVC%, FEV1% in the COPD patients with statistically significant(P<0.01). Conclusion The level of serum HMGB1 in patients with COPD is significantly elevated and is associated with the lung function, indicating that serum HMGB1 level is associated with the severity of chronic obstructive pulmonary disease and has a certain significance of determineing the severity of the disease .

慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)是一种以气流受限为特征的慢性呼吸系统疾病。其气流受限进行性发展,严重者可发展为呼吸衰竭和肺源性心脏病[1]。COPD在全球发病率和死亡率逐年升高,临床表现为气短、呼吸困难、胸闷、慢性咳嗽、咳痰以及其他全身性症状,如疲乏、焦虑和消瘦等,严重影响患者的生活质量。COPD疾病发展过程与多种炎症细胞浸润和炎症细胞因子的作用有关。高迁移率族蛋白B1(high mobility group protein B1,HMGB1)是一种重要的炎症介质和晚期炎症因子,与多种炎症类疾病的发生发展有关[2]。本研究通过检测COPD患者血清中HMGB1的表达水平,分析其与患者肺功能的相关性,探讨血清HMGB1水平与慢性非阻塞性疾病的关系。

1 资料与方法

1.1 一般资料

选取我院2015年1月~2016年5月收治的COPD患者共计73例,其中男48例,女25例,患者年龄52~81岁,平均(69.3±12.9)岁,中位年龄68岁。……
