[摘要] 目的 探討胃小弯全切术与传统的胃癌根治术治疗远端胃癌的效果情况。 方法 分析我院2014年1月～2016年1月普外科收治的104例远端胃癌患者临床资料，依据术式不同进行分组，胃小弯全切术组52例和传统的胃癌根治术组52例。观察两组远端胃癌患者治疗前后Spitzer生活质量指数评分情况，观察两组远端胃癌患者平均淋巴结清除数、1年生存率、复发率情况，观察两组远端胃癌患者并发症发生率情况。 结果 两组远端胃癌患者治疗前活动能力、日常生活、健康感受、家庭支持、生活感受评分均无统计学意义（P>0.05），治疗后传统的胃癌根治术组和胃小弯全切术组远端胃癌患者活动能力、日常生活、健康感受、家庭支持、生活感受评分均高于治疗前，胃小弯全切术组远端胃癌患者治疗后活动能力、日常生活、健康感受、家庭支持、生活感受评分均高于传统的胃癌根治术组，胃小弯全切术组远端胃癌患者平均淋巴结清除数、1年生存率均高于传统的胃癌根治术组，复发率低于传统的胃癌根治术组，胃小弯全切术组远端胃癌患者并发症发生率低于传统的胃癌根治术组，差异有统计学意义（P<0.05）。 结论 胃小弯全切术治疗远端胃癌患者，淋巴结清除效果良好，1年生存率高，复发率低，患者术后生活质量恢复好，并发症发生率低，值得临床推广应用。

Comparison of effects of total resection of lesser curvature and traditional radical gastrectomy in treatment of distal gastric cancer

[Abstract] Objective To explore effects of total resection of lesser curvature and traditional radical gastrectomy in treatment of distal gastric cancer. Methods Clinical data of 104 patients with distal gastric cancer who were treated in department of general surgery in our hospital from January 2014 to January 2016 were analyzed.They were divided into the total resection of lesser curvature group and the traditional radical gastrectomy group according to operative methods，with 52 in each.Spitzer quality of life index scores before and after treatment of patients with distal gastric cancer in two groups were observed.In addition，average number of lymph node dissection，the 1 year survival rate，recurrence rate and the incidence of complications of patients with distal gastric cancer in two groups were observed. Results Activity ability，daily life，feeling of health，family support and feelings of life score before treatment of patients with distal gastric cancer in two groups had no significant difference（P>0.05）.After treatment， activity ability，daily life，feeling of health，family support and feelings of life score of the traditional radical gastrectomy group and the total resection of lesser curvature group were all higher than those before treatment.Activity ability，daily life，feeling of health，family support and feelings of life score of total resection of lesser curvature group after treatment were all higher than those of the traditional radical gastrectomy group.Average number of lymph node dissection and the 1 year survival rate of patients with distal gastric cancer in the total resection of lesser curvature group were both higher than those of the traditional radical gastrectomy group and the recurrence rate was lower than that of the traditional radical gastrectomy group.In addition， the incidence of complications of patients with distal gastric cancer in the total resection of lesser curvature group was lower than that of the traditional radical gastrectomy group，P>0.05.Differences were statistically significant. Conclusion Total resection of lesser curvature in treatment of patients with distal gastric cancer has a good effect on lymph node dissection，a high 1 year survival rate，a low recurrence rate and low incidence of complications.In addition， recovery of quality of life after operation of patients was good.Therefore，it is worthy of clinical promotion and application.
