局部亚低温联合糖皮质激素对急性睾丸扭转复位后睾丸抗氧化能力与精子活力的影响

【摘要】目的:探讨局部亚低温联合糖皮质激素对急性睾丸扭转复位后睾丸抗氧化能力与精子活力的影响。方法:选择2010年1月至2015年3月我院收治的的急性睾丸扭转者80例,按照随机数字法分为两组,各40例,对照组在患者入院后开始口服醋酸地塞米松片,观察组在对照组基础上联合使用局部亚低温处理,比较两组预后SOD、MDA及总抗氧化能力、干预前后两组NO含量及NOS活性,并统计两组干预后精液常规及精子DNA碎片(DFI)指数。结果:干预后,观察组SOD显著高于对照组(P<0.05),而MDA则显著低于对照组(P<0.05),总抗氧化能力以显著高于对照组(P<0.05),干预后观察组NO含量及NOS活性显著低于干预前及干预后对照组(P<0.05),干预后观察组精液中精子密度、精子活动率及精子活力均显著高于对照组(P<0.05),且精子DNA碎片显著少于对照组(P<0.05)。结论:对于急性睾丸扭转者实施局部亚低温联合糖皮质激素处理,能显著提高睾丸抗氧化能力,减轻脂质过氧化反应等缺血缺氧再灌注损伤,减少精子破坏,提高精子活力。

【Abstract】Objectives: To investigate the effect of local mild hypothermia joint sugar temperament hormone on the testicular antioxidant capacity and sperm motility after reset of acute testicular torsion. Methods: 80 patients with acute testicular torsion from January 2010 to March 2015 in our hospital were selected and randomized into two groups, each of 40 cases. The control group orally took dexamethasone acetate tablets, and the observation group received local mild hypothermia on its basis. The SOD, MDA and total antioxidant capacity, NO content and NOS activity before and after the intervention were compared to summarize the semen and sperm DNA fragmentation (DFI) index. Results: After the intervention, SOD in the observation group was significantly higher than control group (P<0.05); MDA was significantly lower than control group (P<0.05); total antioxidant capacity was significantly higher control group (P<0.05). After intervention, the NO content and NOS activity was significantly lower than pre-intervention in observation group and otherwise in control group (P<0.05). After intervention, the semen sperm concentration, sperm motility and sperm motility in observation group was significantly higher than control group (P<0.05), while sperm DNA fragmentation was significantly less than control group (P<0.05). Conclusions: Local mild hypothermia joint sugar temperament hormone can significantly improve the testicular antioxidant capacity and reduce the lipid peroxidation and other hypoxia-ischemia-reperfusion injury, thusly reducing sperm damage and improving sperm motility.

睾丸扭转主要是因为精索发生扭转导致,其可发生于任何年龄阶段,其中急诊科收治的超过25%阴囊方面急诊为睾丸扭转,如未能得到及时救治,需要切除睾丸的比例甚至超过50%[1]。其好发于新生儿期及青春期者,占青春期睾丸急诊的50%以上比例,本病早期极易误诊,已经成为中青年尤其是青春期男性失去睾丸的最主要原因,进而导致男性不育症的发生。临床经验证实[2],对于怀疑睾丸扭转者甚至对于不能排除诊断者,应及时手术治疗,以最大限度减少睾丸缺血缺氧时间,提高患者预后。尽管对于拟者已经明确尽快实施手术治疗的价值,但部分患者入院后因主观犹豫等,未能及时签署手术和麻醉同意书,以及重要的术前检查,如超声、凝血常规、心电图等亦需要一定时间,为更好的挽救患者病变睾丸,提高治疗效果,临床急需一种在本阶段内更好的减少睾丸缺血缺氧的方法[3]。……
