创造性故事疗法对老年期痴呆病人认知功能与生活质量的影响

摘要:[目的]探讨创造性故事疗法对老年期痴呆病人认知功能与生活质量的影响。 [方法]选择轻中度老年期痴呆病人31例实施创造性故事疗法,干预前后分别测量病人的认知功能与生活质量,并通过自制满意度调查表了解病人对创造性故事疗法干预的感受。[结果]干预前后病人生活质量量表(QOL-AD)评分比较差异有统计学意义(P<0.05);MMSE量表评分比较差异无统计学意义,但是干预后评分较干预前评分有所提高;病人对创造性故事疗法的整体满意度为90.3%。[结论]创造性故事疗法有助于维持老年期痴呆病人的认知功能,改善其生活质量。

AbstractObjective:To probe into the influence of creative story therapy on cognitive function and quality of life of senile dementia patients.Methods:A total of 31 patients with mild and moderate senile dementia were selected and received creative story therapy,and to measure the cognitive function and life quality of patients before and after the intervention,and to know about the feelings of patients to creative story therapy by using the self-made satisfaction table.Results:The difference of quality life table(QOL-AD) score has statistical significance before and after the intervention(P<0.05);the difference in MMSE score haven’t statistical significance,but the score was improved after the intervention;the overall satisfaction degree of patients on creative story therapy was 90.3%.Conclusion:Creative story therapy could help to maintain the cognitive function of senile dementia patients and improve their quality of life.

痴呆是一种以认知功能缺损为核心症状的获得性智能损害综合征,其认知损害可涉及记忆、学习、定向、理解、判断、计算、语言、视空间等功能,智能损害程度足以干扰日常生活能力或社会职业功能;在其病程某一阶段常伴有精神、行为、人格异常;具有慢性和进行性的特点[1]。老年期痴呆泛指发生于65岁以后这个年龄阶段的各种痴呆,包括阿尔茨海默病(Alzheimer’s disease,AD)、血管性痴呆(vascular dementia,VD)、混合性痴呆(mixed dementia,MD)等类型[2]。随着我国老龄化进程的不断加深,老年期痴呆的患病率也呈逐年上升趋势,成为继癌症、心脏病、脑血管病之后引起老年人死亡的第4大病因[3],已成为当今社会必须面对的最大挑战之一。目前,对老年期痴呆尚缺乏特异的治疗手段,所以延缓老年痴呆疾病的进程,提高病人的生活质量就显得特别重要[4]。国外有学者提出,当理性的语言失败时,创造性的表达为老年痴呆病人提供了一种新的沟通渠道,它提供给老年痴呆病人和他们的照顾者表达自我、分享喜悦、共同成长的机会[5]。……
