[摘要]目的：研究滚针疗法联合透明质酸修复生物膜治疗面部皮炎的临床疗效。方法：选取2022年1月-2023年6月眉山市人民医院收治的198例面部皮炎患者，用简单随机化法分为对照组、联合组，每组99例。对照组接受透明质酸生物膜治疗，联合组在对照组基础上使用滚针治疗。比较两组的临床疗效、治疗前后的皮损面积和临床症状评分、皮肤生理指标、VISIA图像观察、生活质量、不良反应及复发率。结果：联合组临床疗效的总有效率高于对照组（P＜0.05）；治疗后，两组皮损面积和临床症状评分均显著减小，且治疗后联合组的皮损面积和临床症状评分均小于对照组（P＜0.05）；治疗后，两组黑色素差异无统计学意义（P＞0.05），但皮肤含水量增加，经皮失水量减少，且治疗后联合组的皮肤含水量高于对照组、经皮失水量低于对照组（P＜0.05）；治疗后两组斑点、毛孔、红色区均减小，且联合组斑点、毛孔、红色区小于对照组（P＜0.05）；治疗后，两组皮肤病生活质量指数（DLQI）维度评分及总分减小，且联合组DLQI维度评分及总分低于对照组（P＜0.05）；两组不良反应无统计学意义（P＞0.05）；随访6个月，联合组的复发率小于对照组，但差异无统计学意义（χ2=3.194，P=0.074）。结论：滚针疗法联合透明质酸修复生物膜治疗面部皮炎，不仅能够提高治疗效果，改善皮肤生理指标和生活质量，还可以降低复发率且安全。

Clinical Effect of Needle-rolling Therapy Combined with Hyaluronic Acid Dressing in the Treatment of Facial Dermatitis

Abstract： Objective" To study the clinical effect of needle-rolling therapy combined with hyaluronic acid dressing in the treatment of facial dermatitis. Methods" A total of 198 patients with facial dermatitis who were admitted to Meishan People's Hospital from January 2022 to June 2023 were selected for the study. They were randomly assigned to the control group and the combination group， with 99 cases in each group. The control group was treated with hyaluronic acid biomask， and the combination group was given needle-rolling therapy on this basis. Comparisons were made between the two groups on clinical efficacy， lesion areas， clinical symptom scores， skin physiological indicators， VISIA images and quality of life before and after treatment， adverse reactions， and recurrence rate. Results" The total clinical effective rate of the combination group was higher than that of the control group （P＜0.05）. After treatment， the area of skin lesions and clinical symptom scores decreased significantly in both groups， and were smaller and lower in the combination group than in the control group （P＜0.05）. After treatment， melanin was comparable between the two groups （P＞0.05）. However， stratum corneum water content increased， and transepidermal water loss decreased in both groups. In addition， stratum corneum water content was higher and transepidermal water loss was less in the combination group than in the control group （P＜0.05）. After treatment， spots， pores and red areas decreased in both groups， and were smaller in the combination group than in the control group （P＜0.05）. After treatment， the Dermatology Life Quality Index （DLQI） scores in different dimensions and total score decreased in both groups. Meanwhile， the combination group got lower scores than the control group （P＜0.05）. Adverse reactions in the two groups were similar （P＞0.05）. The recurrence rate of the combination group during 6 months of follow-up was lower than that of the control group ， but the difference was not statistically significant （χ2=3.194， P=0.074）. Conclusion" Applying needle-rolling therapy combined with hyaluronic acid dressings to treat facial dermatitis can not only improve the treatment effect， skin physiological indicators and quality of life but also reduce the recurrence rate， with safety.
