正畸联合牙周夹板在牙周炎所致前牙扇形移位治疗中的美学效果

[摘要]目的探討在牙周炎所致前牙扇形移位患者中应用正畸联合牙周夹板治疗的效果。方法选择2021年5月-2023年5月我院收治的70例牙周炎所致前牙扇形移位患者为研究对象，根据治疗方案不同分为对照组的研究组，各35例。对照组使用牙周夹板治疗，研究组使用正畸联合牙周夹板治疗，比较两组临床疗效、牙周状况、炎性因子水平、牙齿美观度及咀嚼功能。结果研究组治疗总有效率为97.14%，高于对照组的80.00%（P<0.05）；研究组治疗后AL、SBI、PLI、PD及牙齿松动度均优于对照组（P<0.05）；研究组治疗后CRP、MCP-1、sICAM-1、IL-6水平均低于对照组（P<0.05）；研究组治疗后牙齿色泽、牙齿形状、与邻牙关系评分均高于对照组（P<0.05）；研究组治疗后咀嚼功能评分高于对照组（P<0.05）。结论在牙周炎所致前牙扇形移位患者中应用正畸联合牙周夹板治疗的效果良好，可协同性抑制炎症，改善患者的牙周状况，有利于提高牙齿美观度及咀嚼功能，值得临床应用。

Aesthetic Effect of Orthodontics Combined with Periodontal Splint in the Treatment of Anterior Teeth Fan-shaped Displacement Caused by Periodontitis

[Abstract]Objective To explore the aesthetic effect of orthodontic combined with periodontal splints in the treatment of anterior fan-shaped displacement caused by periodontitis. Methods A total of 70 patients with anterior teeth fan-shaped displacement caused by periodontitis admitted to our hospital from May 2021 to May 2023 were selected as the research objects. According to different treatment schemes， they were divided into the study group of the control group， with 35 patients in each group. The control group was treated with periodontal splints， and the study group was treated with orthodontics combined with periodontal splints. The clinical efficacy， periodontal status， inflammatory factor levels， tooth aesthetics and masticatory function were compared between the two groups. Results The total effective rate of treatment in the study group was 97.14%， which was higher than 80.00% in the control group （P<0.05）. After treatment， AL， SBI， PLI， PD and tooth mobility in the study group were better than those in the control group （P<0.05）. After treatment， the levels of CRP， MCP-1， sICAM-1 and IL-6 in the study group were lower than those in the control group （P<0.05）. The scores of tooth color， tooth shape and relationship with adjacent teeth in the study group were higher than those in the control group （P<0.05）. The masticatory function score of the study group after treatment was higher than that of the control group （P<0.05）. Conclusion Orthodontic combined with periodontal splint therapy has a good effect in patients with anterior teeth fan-shaped displacement caused by periodontitis， which can synergistically inhibit inflammation， improve the periodontal status of patients. Meanwhile， it is conducive to improving tooth aesthetics and chewing function， which is worthy of clinical application.

牙周炎（periodontitis）在口腔科疾病中较为常见，常会累及牙龈、牙周膜、牙骨质等支持组织，导致前牙发生移位、扭转、变形等，若未及时治疗还可导致牙齿脱落，给牙齿美观度及咀嚼功能造成不利影响[1]。牙周夹板是临床治疗牙周炎所致前牙扇形移位的有效手段，尽管在改善相关症状方面具有一定效果，但在美学效果方面的改善作用较弱[2]。口腔正畸可将散开的前牙内收、排齐，促使牙槽骨及牙周膜重建，在提升治疗效果及牙齿美观度方面具有独特优势[3]。基于此，本研究旨在探究正畸联合牙周夹板在牙周炎所致前牙扇形移位治疗中的美学效果，现报道如下。
