[摘要]目的：探讨地氯雷他定辅助异维A酸治疗寻常痤疮的疗效。方法：选取2019年4月-2020年3月在笔者医院确诊并治疗的174例寻常痤疮患者为研究对象，按照随机数字表法分为联合治疗组（地氯雷他定和异维A酸共同治疗）和对照组（仅用异维A酸治疗），每组87例。采用酶联免疫法（ELISA）检测患者治疗前后炎症因子白细胞介素1（IL-1）、白细胞介素6（IL-6）、白细胞介素8（IL-8）、肿瘤坏死因子（TNF-α）表达水平，并用皮肤损伤改善程度评价治疗效果，统计病情恢复情况。结果：治疗后，两组血清IL-1、IL-6、IL-8、TNF-α表达水平比治疗前降低（P<0.05），联合治疗组血清IL-1、IL-6、IL-8、TNF-α表达的水平低于对照组（P<0.05）;治疗后，联合治疗组痊愈例数多于对照组，总有效率96.6%，高于对照组的87.4%。治疗后，联合治疗组的结痂时间较对照组短（P<0.05）。结论：地氯雷他定联合小剂量异维A酸治疗寻常痤疮能提高痊愈率，具有较好的疗效，有效降低血清IL-1、IL-6、IL-8、TNF-α表达水平。

Application of Desloratadine Combined with Isotretinoin in the Treatment

of Acne Vulgaris

Abstract： Objective  To evaluate the adjuvant value of desloratadine in the treatment of acne with isotretinoin. Methods 174 cases of acne patients diagnosed and treated in our hospital from April 2019 to March 2020 were selected and divided into the combined treatment group （desloratadine and isotretin） according to the random number table method. The control group included 87 patients （treated with isotretinoin alone）. The expression levels of inflammatory factors interleukin-1 （IL-1）， interleukin-6 （IL-6）， interleukin-8 （IL-8） and tumor necrosis factor （TNF-α）were detected by ELISA before and after treatment， and the treatment effect was evaluated by the degree of improvement of skin injury， and the condition recovery was counted. Results  The expression levels of IL-1， IL-6， IL-8 and TNF- in the two groups were lower after treatment than before （P<0.05）. After treatment， the levels of serum IL-1， IL-6， IL-8 and TNF- in the combined treatment group were lower than those in the control group （P<0.05）. The total effective rate （96.6%） was higher than that of the control group （87.4%）. The scab time of the combined treatment group was shorter than that of the control group （P<0.05）. Conclusion  Desloratadine combined with small dose of isotretinoin in the treatment of acne can improve the recovery rate， has A good effect， and effectively reduces the serum IL-1， IL-6， IL-8， TNF-αexpression levels.

寻常痤疮是累及毛囊皮脂腺的一种慢性炎症性皮肤病，临床症状表现为粉刺、丘疹及脓疱等，发病部位一般为面颈部、胸背部、肩膀和上臂[1]。目前，20～30岁的成年人患病率为65%，而青少年发病率高达90%[2-3]。寻常痤疮患者在接受治疗后或多或少会有皮肤损伤残留，给患者及家属精神上造成严重影响。目前关于治疗寻常痤疮的研究大多聚焦于异维A酸、激光治疗，但激光治疗花费相对较大，而单独使用异维A酸疗效尚不满意。本研究旨在了解地氯雷他定与异维A酸在尋常痤疮治疗中的联合应用价值。

1  资料和方法

1.1 一般资料：经笔者医院伦理委员会同意，选取2019年4月-2020年3月在笔者医院确诊并治疗的174例寻常痤疮患者为研究对象，按照随机数字表法分为两组。联合治疗组：87例，采用药物地氯雷他定和异维A酸联系治疗;对照组：87例，采用异维A酸治疗。……
