摘 要 目的：探索新型的中药临床药师的工作模式，为合理使用中药饮片和中成药提供服务。方法：探索建立“智能化事前审方、医、药联合门诊、药师独立门诊、专科化药学查房、临床药师参与疑难病患者多学科协作诊疗（MDT）、跨机构审方等中药临床药学服务模式”，提升中药饮片应用质量。结果：中药临床药学服务模式能有效促进中药合理用药。结论：中药临床药学工作和服务模式可供同行借鉴。

Discussion on the work and service mode of clinical pharmacists of traditional Chinese medicine in secondary Chinese medicine hospitals

ABSTRACT Objective： To explore a new type of work mode of clinical pharmacists of traditional Chinese medicine（TCM） and provide services for the rational use of Chinese medicine decoction pieces and Chinese patent medicine. Methods： We explored to establish some service modes including intelligent preview prescription system， medical and pharmaceutical joint clinics， independent pharmacist clinics， specialized pharmacy ward rounds， clinical pharmacists participating in multidisciplinary collaborative diagnosis and treatment （ MDT） of intractable case and so on， so as to improve the application quality of TCM. Results： These modes could effectively promote the rational use of TCM. Conclusion： These work and service modes can be used for reference by peers.

中药临床药学是指在中医药理论指导下，以临床为服务对象，研究中药及制剂与人体相互作用的安全性、有效性及应用规律的一门综合性学科，是临床药学的一个新分支，其核心内容是中药治疗的合理性[1]。相比于西医临床药学，中药临床药学的发展相对比较缓慢，基本处于起步和探索阶段[2]。

杨浦区中医医院（以下称“我院”）作为一家二级甲等中医医院，中药饮片及中成药的使用比例较高，存在很多不合理用药的情况[3]，如何使其合理应用，是中药临床药师的主要任务。经过3年多的药学服务工作探索和创新，我院临床药师团队创建了一套比较成熟的中药临床药师服务模式，取得了可喜的进步。本文就此作一总结，与同行分享。

1 临床药师服务内容

我院现有专职临床药师7名，直接参与临床工作，促进了临床中西药物的合理应用，主要工作内容如下。……
