摘 要 目的：瑞芬太尼复合七氟醚控制性降压麻醉在鼻内窥镜手术中的应用价值分析。方法：选取80例行鼻内窥镜手术患者，采用随机数字抽样法划分为观察组（n=40）和对照组（n=40），对照组接受静吸复合全身麻醉，观察组在此基础上采用瑞芬太尼复合七氟醚控制性降压。对比两组患者在手术开始后10 min、30 min和手术结束后的平均动脉压控制效果、不良反应发生率和治疗满意度。结果：观察组平均动脉压控制效果优于对照组，不良反应发生率低于对照组，治疗满意度高于对照组（均P<0.05）。结论：瑞芬太尼复合七氟醚在鼻内窥镜手术中的控制性降压麻醉效果良好，可以降低患者的不良反应发生率，提高患者的满意度。

The application of remifentanil combined with sevoflurane for controlled hypotensive anesthesia in nasal endoscopic surgery

ABSTRACT Objective： To analyze the value of the application of remifentanil compound sevoflurane controlled hypotensive anesthesia in nasal endoscopic surgery. Methods： Eighty patients undergoing nasal endoscopy were selected and divided into an observation group （n=40） and a control group （n=40） by random digital sampling method. The control group received general anesthesia with static inhalation compound， and the observation group used controlled hypotension with remifentanil combined with sevoflurane on this basis. The mean arterial pressure control， the incidence of adverse effects and the treatment satisfaction were compared between the two groups at 10 min and 30 min after the start of surgery and at the end of surgery. Results： The mean arterial pressure control effect was better， the treatment satisfaction was higher and the incidence of adverse effects was lower in the observation group than the control group （all P<0.05）. Conclusion： The controlled hypotensive anesthesia with remifentanil combined with sevoflurane in nasal endoscopic surgery is effective and can reduce the incidence of adverse reactions and improve patient satisfaction.

鼻內窥镜手术是一种操作难度较高、较复杂的手术[1]。一般在患者进行该手术前，需要进行静息复合全身麻醉来帮助患者在手术中减小创口、降低并发症，但是在术中患者有较大可能出现应激反应[2]，因此需要对患者采取控制性降压来提高手术的治疗效果。本文主要研究瑞芬太尼复合七氟醚对在鼻内窥镜手术中控制性降压麻醉的使用效果，现报道如下。

1 资料与方法

1.1 一般资料

选取我院2018年6月—2020年6月进行鼻内窥镜手术的患者80例，采用随机数字抽样法划分为观察组和对照组各40例。观察组：男25例，女15例；年龄22～60岁，平均年龄（42.81±9.25）岁；体质量45～85kg，平均体质量（65.81±9.25）kg。对照组：男26例，女14例；年龄22～59岁，平均年龄（42.78±9.05）岁；体质量45～84 kg，平均体质量（65.14±9.08）kg。两组患者一般资料对比无显著性差异，具有可比性（P>0.05）。
