摘 要 目的：探索中医临床药师门诊服务的新模式和为患者提供专病化服务的新途径。方法：开设中药临床药师-中医联合门诊，为积聚病（乳腺肿瘤）患者提供临床诊疗和中药学服务。结果：通过联合门诊，对积聚病（乳腺肿瘤）患者用药更具个体化，药学监护更直接，有效提高患者满意度。结论：中药临床药师-中医联合门诊有利于提升中医用药的合理性、规范性和药学监护的个体性，有利于提升医师与药师业务的融合性。

Exploration of the service mode of joint-outpatient service between TCM clinicians and pharmacists*

ABSTRACT Objective： To explore a new mode of outpatient service for clinical pharmacists of traditional Chinese medicine （TCM） and a new way to provide specialized services for patients. Methods： A joint-outpatient service between TCM clinicians and pharmacists was set up to provide clinical diagnosis and treatment and Chinese pharmacy services for patients with accumulation disease （breast tumor）. Results： Through the joint-outpatient service， the medication for the patients was more individualized， and the pharmaceutical care was more direct， which could effectively improve the patient satisfaction. Conclusion： The rationality and standardization of TCM medication， the individuality of pharmaceutical care and the integration between physicians and pharmacists can be all improved through the join-outpatient service.

中医药是祖国传统文化隗宝，由于中医药自身的独特性、复杂性、中医临床应用的经验性、单人单方的特殊性，再加之与现代医学理论结合，中西药联合使用的过程中，中药使用的安全性渐渐显现出其重要[1]。保证临床中药应用的安全、有效，防止和最大限度地减少中药的不良反应，促进临床合理用药，保障患者用药安全，是今后中药临床药学领域的工作重点。由于我国中药临床药学刚刚起步，中药临床药师所开展的药学服务没有固定的模式可以参考，本文根据我院开展的药学门诊：中药临床药师-中医联合门诊的工作实践，探索药学门诊工作服务新模式，探索为积聚病（乳腺肿瘤）患者提供的专病化服务新途径，进行专病收集病例汇总分析，与广大中药临床药师共同学习探讨，以期提高中药临床药学的服务水准。……
