安罗替尼联合同步放射治疗局部晚期非小细胞肺癌患者的临床研究

【摘要】 目的:分析安羅替尼联合同步放射治疗对局部晚期非小细胞肺癌患者疗效及安全性的影响。方法:选取2020年3月-2021年2月本院收治的80例局部晚期非小细胞肺癌患者,采用随机数字表法将其分为对照组(n=40)和观察组(n=40)。对照组给予安慰剂联合同步放射治疗,观察组给予安罗替尼联合同步放射治疗。比较两组治疗效果及治疗前后免疫功能、血清肿瘤标记物水平、生活质量核心问卷(QLQ-C30)、肺癌专项量表(QLQ-LC13)评分,并比较两组毒副作用发生情况。结果:观察组客观缓解率(ORR)、疾病控制率(DCR)均明显高于对照组(P<0.05)。治疗后,两组CD8+水平较治疗前均下降,CD3+、CD4+、CD4+/CD8+水平较治疗前均上升,且观察组均优于对照组(P<0.05)。治疗后,两组血清CEA、CYFRA21-1、VEGF、MMP9较治疗前均下降,且观察组均低于对照组(P<0.05)。治疗后,两组QLQ-C30评分较治疗前均上升,QLQ-LC13评分较治疗前均下降,且观察组均优于对照组(P<0.05)。两组毒副反应发生率比较,差异无统计学意义(P>0.05)。结论:安罗替尼联合同步放射可提高局部晚期非小细胞肺癌患者疗效及免疫功能,还可控制其肿瘤标志物水平,改善患者预后。

[Abstract] Objective: To analyze the effect of Anlotinib combined with synchronous radiotherapy on the efficacy and safety of patients with locally advanced non-small cell lung cancer. Method: A total of 80 patients with locally advanced non-small cell lung cancer treated in our hospital from March 2020 to February 2021 were selected. They were divided into the control group (n=40) and the observation group (n=40) according to the random number table method. The control group was given placebo combined with synchronous radiotherapy, and the observation group was given Arotinib combined with synchronous radiotherapy. The therapeutic efficacy and immune function, serum tumor marker level, quality of life core questionnaire (QLQ-C30) and lung cancer special scale (QLQ-LC13) scores of two groups before and after treatment were compared, and the incidence of toxic and side effects of two groups were compared. Result: The objective remission rate (ORR) and disease control rate (DCR) in the observation group were significantly higher than those in the control group (P<0.05). After treatment, the levels of CD8+ of two groups decreased compared with those of before treatment, and the levels of CD3+, CD4+, CD4+/CD8+ increased compared with those of before treatment, and those of the observation group were better than those of the control group (P<0.05). After treatment, the levels of serum CEA, CYFRA21-1, VEGF and MMP9 of two groups were lower than those of before treatment, and the observation group were lower than those of the control group (P<0.05). After treatment, the QLQ-C30 scores of two groups increased compared with those of before treatment, and the QLQ-LC13 scores decreased compared with those of before treatment, and those of the observation group were better than those of the control group (P<0.05). There was no statistically significant difference in the incidence of various side effects between two groups (P>0.05). Conclusion: Anlotinib combined with synchronous radiotherapy can improve the efficacy and immune function of patients with locally advanced non-small cell lung cancer, and can also control the level of tumor markers and improve the prognosis of patients.

局部晚期非小细胞肺癌(LANSCLC)是临床上常见于胸外科的一种恶性肿瘤,占NSCLC的60%~70%,占全部肺癌的50%以上[1]。LANSCLC发病机制尚不清晰,临床上常表现为消瘦、咳嗽、气短、乏力、咯血、胸痛等,伴有免疫功能下降表现,严重影响患者生命质量,且临床表现特异性低,导致患者就诊时多处于WHO分期ⅢA、ⅢB期,错过最佳治疗时间[2]。……
