【摘 要】目的：探究在初产妇中应用医院—社区—家庭一体化护理模式对母乳喂養的干预效果。方法：选择在我院产检和分娩的初产妇98例作为研究对象，将初产妇按随机数字表法均分两组，对照组给予常规护理，观察组给予常规护理+医院—社区—家庭一体化护理，比较两组初产妇产后1个月、3个月、6个月内的母乳喂养率，6个月内哺乳期乳腺疾病及婴儿喂养不良反应的发生率。结果：观察组初产妇产后1个月、产后3个月、产后6个月母乳喂养率明显高于对照组，差异显著（P<0.05）；观察组初产妇产后6个月内哺乳期乳腺疾病发生率低于对照组，差异显著（P<0.05）；观察组产后6个月内婴儿喂养不良反应的发生率低于对照组，差异显著（P<0.05）。结论：在初产妇护理中采用医院—社区—家庭一体化护理模式可提高初产妇6个月内母乳喂养率，降低哺乳期乳腺疾病发生率，降低婴儿喂养不良反应发生率，护理价值高，值得临床应用推广。

Study on the effect of hospital-community-family integrated nursing model on breastfeeding intervention of primipara

【Abstract】Objective： To explore the effect of applying hospital-community-family integrated nursing model on breastfeeding intervention in primiparas. Methods： 98 primiparas who had undergone obstetric examination and delivery in our hospital were selected as the research objects. The primiparas were equally divided into two groups according to a random number table. The control group was given routine care， and the observation group was given routine care+hospital-community-family integration Nursing， to compare the breastfeeding rates of the two groups of primiparous women within 1 month， 3 months， and 6 months after delivery， the incidence of breast disease during lactation and the incidence of infant feeding adverse reactions within 6 months. Results： The breastfeeding rates of primiparas in the observation group were significantly higher than those in the control group at 1 month， 3 months， and 6 months after delivery， and the difference was significant （P<0.05）； The incidence of breast-feeding disease in the observation group within 6 months after delivery was significantly lower than that in the control group， and the difference was significant（P<0.05）； The incidence of infant feeding adverse reactions in the observation group was lower than that in the control group within 6 months after delivery， and the difference was significant（P<0.05）.Conclusion： The hospital-communityfamily integrated nursing model in primipara care can increase the breastfeeding rate of primiparas within 6 months， reduce the incidence of breast disease during lactation， and reduce the incidence of infant feeding adverse reactions. The nursing value is high and worthwhile Clinical application promotion.

初产妇由于对分娩知识掌握不全，不了解母乳喂养对自身、婴儿的重要性，加上分娩疼痛引起的焦慮、抑郁情绪等，导致其产后无法快速适应角色转变，影响母乳喂养率[1] 。新手妈妈哺乳期保健知识匮乏，新生儿养育体系尚未建立，对正常的泌乳尚且出现手忙脚乱，哺乳时又出现喂养姿势不当，加上婴儿频繁吸吮造成的乳腺刺激，容易导致乳头皲裂、乳胀、乳腺炎等乳腺疾病[2-4]。……
