银离子藻酸盐治疗术后伤口脂肪液化的效果

【摘要】 目的:觀察银离子藻酸盐治疗术后伤口脂肪液化的效果。方法:选择2017年7月-2019年10月笔者所在医院术后伤口脂肪液化患者120例,根据就诊单双号分为试验组和对比组,各60例。对比组采用常规消毒后引流措施,试验组在对比组基础上加用银离子藻酸盐。观察两组新鲜肉芽组织形成时间、伤口愈合时间、换药次数、治疗费用及愈合情况。结果:试验组新鲜肉芽组织形成时间、伤口愈合时间、换药次数、治疗费用分别为(2.9±0.6)d、(9.8±0.7)d、(2.1±0.4)次、(609.8±53.2)元,均优于对比组的(4.3±0.8)d、(15.3±1.2)d、(4.2±0.9)次、(932.1±57.5)元,差异均有统计学意义(P<0.05)。试验组伤口愈合情况(甲级60例)优于对照组(甲级48例,乙级8例,丙级4例),差异有统计学意义(P<0.05)。结论:银离子藻酸盐能够促进伤口脂肪液化处肉芽组织生长,缩短伤口愈合时间,减少换药次数,减轻患者的经济负担,促进伤口良好愈合。

[Abstract] Objective: To observe the effects of Silver Ion Alginate on fat liquefaction of wound after operation. Method: A total of 120 patients with fat liquefaction of wound after operation in our hospital from July 2017 to October 2019 were selected. They were divided into the experimental group and the comparison group according to the single and double numbers of treatment, 60 cases in each group. The comparison group was drained after conventional disinfection, and the experimental group was treated with Silver Ion Alginate on the basis of the comparison group. And the formation time of fresh granulation tissue, wound healing time, number of dressing changes, treatment cost and healing were observed between the two groups. Result: The formation time of fresh granulation tissue, wound healing time, number of dressing changes and treatment cost in the experimental group were (2.9±0.6) days, (9.8±0.7) days, (2.1±0.4) times and (609.8±53.2) yuan, which were better than (4.3±0.8) days, (15.3±1.2) days, (4.2±0.9) times and (932.1±57.5) yuan of the comparison group, and the differences were statistically significant (P<0.05). The time of formation of fresh granulation tissue, the time of wound healing, the number of dressing changes, and the total cost of treatment in the observation group were lower than those in the comparison group (P<0.05). Wound healing in the experimental group (60 cases of grade A) was better than that of the comparison group (48 cases of grade A, 8 cases of grade B, and 4 cases of grade C), and the difference was statistically significant (P<0.05). Conclusion: Silver Ion Alginate can promote the growth of granulation tissue in fat liquefaction of wound, shorten the wound healing time, and reduce the number of dressing changes, reduce the economic burden on patients, and promote wound healing.

腹部手术后5~7 d常出现伤口脂肪液化,反复渗出影响愈合效果[1]。肥胖和电刀是切口液化的重要因素,一旦处理不当可导致感染而影响切口愈合[2]。若渗出量较少且切口没有裂开,应挤压切口以促使渗液排出[3]。银离子藻酸盐敷料是一种具有抗菌作用的高吸收性非织造敷料,结构柔软[4]。本文主要观察银离子藻酸盐治疗术后伤口脂肪液化的效果,报道如下。
1 资料与方法
1.1 一般资料
选取2017年7月-2019年10月笔者所在医院术后伤口脂肪液化患者120例。纳入标准:(1)年龄>18岁;(2)术后出现伤口脂肪液化,拆线后迁延不愈1周以上;(3)腹部切口<12 cm;(4)中、大量渗出液;(5)凝血功能正常。排除标准:(1)营养不良;(2)糖尿病;(3)低蛋白血症;(4)长期使用免疫抑制剂;(5)肿瘤多处转移。根据就诊单双号分为试验组与对比组,各60例。试验组男20例,女40例;年龄49~69岁,平均(52.1±3.2)岁。对比组男22例,女38例;年龄40~70岁,平均(52.3±3.5)岁。两组一般资料比较,差异均无统计学意义(P>0.05),具有可比性。
1.2 方法
对比组采取常规消毒后置橡皮管引流,每日换药,创面肉芽组织生长后行二次缝合等措施。试验组消毒皮肤后采用生理盐水冲洗,拆除缝线,清理伤口,采用银离子藻酸盐抗菌敷料[Advanced Medical Solutions Ltd.生产,国食药监械(进)字2014第3643633号]覆盖创面,每24小时更换1次。封……
