基于核转录因子-κB信号通路探讨针灸四关穴治疗偏头痛的机制

摘要 目的:基于核轉录因子-κB信号通路(NF-κB)探讨针灸四关穴治疗偏头痛疗效。方法:选取2018年2月至2019年2月广州中医药大学祈福医院收治的偏头痛患者114例作为研究对象,按照随机数字表法随机分为对照组和观察组,每组57例。对照组采用常规西药治疗,观察组在常规西药治疗的基础上加用针灸四关穴治疗,均治疗4周。比较2组症状积分、临床疗效、NF-κB信号通路及其相关指标变化。结果:治疗后2组降钙素原基因相关肽(CGRP)、VAS评分、症状持续时间、发作频率、NF-κB、环氧合酶抑制剂(COX-2)、白细胞介素-1β(IL-1β)、一氧化氮合酶(iNOS)、肿瘤坏死因子-α(TNF-α)、大脑前动脉(ACA)、大脑中动脉(MCA)、大脑后动脉(PCA)、椎动脉(VA)、基底动脉(BA)平均血流速度均较治疗前下降,差异均有统计学意义(P<0.05),而β-内啡肽(β-EP)和5-羟色胺(5-HT)均较治疗前升高,差异均有统计学意义(]P<0.05),且观察组优于对照组,差异有统计学意义(P<0.05)。观察组总有效率为87.72%,高于对照组的71.93%,差异有统计学意义(P<0.05)。结论:四关穴能改善NF-κB信号通路,改善大脑动脉血流速度,从而改善偏头痛症状。

Mechanism of Acupuncture and Moxibustion at Siguan Point for Migraine Based on Nuclear Transcription Factor-Kappa B Signaling Pathway

Abstract Objective:To explore the efficacy of acupuncture at Siguan acupoint in the treatment of migraine based on nuclear factor-κ B signal pathway (NF-κ B).Methods:A total of 114 patients with migraine treated in Qifu Hospital of Guangzhou University of Traditional Chinese Medicine from February 2018 to February 2019 were randomly divided into a control group (n = 57) and an observation group (n = 57).The control group was treated with routine Western medicine,and the observation group was treated with acupuncture at Siguan point for 4 weeks.The symptoms,clinical efficacy,NF-κ B signal pathway and related indexes were compared between the 2 groups.Results:After treatment,procalcitonin gene-related peptide (CGRP),VAS score,duration,seizure frequency,NF-κ B,cyclooxygenase inhibitor (COX-2),interleukin-1 β (IL-1 β),nitric oxide synthase (iNOS),tumor necrosis factor-α (TNF-α),anterior cerebral artery (ACA),middle cerebral artery (MCA),posterior cerebral artery (PCA),vertebral artery (VA),and the mean blood flow velocity of basilar artery (BA) were lower than those before treatment (P<0.05).The levels of β-endorphin (β-EP) and 5-hydroxytryptamine (5-HT) were higher than those before treatment(P<0.05),and the observation group was better than the control group (P<0.05).The total effective rate of the observation group was 87.72%,which was higher than that of the control group (71.93%).The difference was statistically significant (P<0.05).Conclusion:Acupuncture and moxibustion at Siguan point can improve the signal pathway of NF-kappa B and the blood flow velocity of cerebral artery,so as to improve the symptoms of Migraine.

1.8 疗效判定标准 采用尼莫地平法评价完成治疗后总疗效,显效为症状积分减少>75%,有效为症状积分减少30%~74%,无效为症状积分减少<30%[9]。总有效率=(显效+有效)例数/总例数×100%。

1.9 统计学方法 采用SPSS 22.0统计软件对研究数据进行统计分析,计量资料用均数±标准差(]±s)进行表示,采用t检验;计数资料用百分比(%)表示,采用χ2检验,以P<0.05为差异有统计学]意义。

2 结果

2.1 2组患者血液学相关指标比较 2组患者完成治疗后CGRP水平较治疗前均显著下降,差异有统计学意义(P<0.05),而β-EP、5-HT较治疗前均显著升高,差异有统计学意义(P<0.05),且观察组以上指标均显著优于对照组,差异有统计学意义(P<0.05)。见表1。

2.2 2组患者症状积分比较 2组患者完成治疗后症状积分VAS评分、持续时间、发作频率、伴随症状较治疗前均显著下降,差异有统计学意义(P<0.05),且观察组以上指标均显著低于对照组,差异有统计学意义(P<0.05)。见表2。
