浮针治疗颈椎病临床疗效的Meta分析

摘要:目的  基于Meta分析系統评价浮针治疗颈椎病的临床疗效,为临床治疗颈椎病提供参考。方法  以中国生物医学文献数据库(CBM)、中国知识资源总库(CNKI)、中国学术期刊数据库(万方数据)、中文科技期刊数据库(维普网)、PubMed、Embase、Cochrane Library 为检索数据库,收集建库至2020年2月浮针治疗颈椎病的随机对照试验(RCT),采用Cochrane5.1.0偏倚风险评估工具对纳入文献进行质量评价,并用RevMan5.3进行Meta分析。结果  共纳入文献15篇,总样本量1284例。试验组总有效率(OR=2.88,95%CI[1.92,4.32],P<0.05),治愈率(OR=2.11,95%CI[1.43,3.13],P<0.05),即时起效时间(MD=-129.71,95%CI[-138.38,-121.04],P<0.05),总体VAS评分(MD=-1.22,95%CI[-2.28,-0.15],P<0.05);亚组分析:VAS评分(神经根型)(MD=-1.79,95%CI[-3.33,-0.25],P<0.05),VAS评分(颈型)(MD=-0.41,95%CI[-0.61,-0.20],P<0.05);均优于对照组,且组间比较具有统计学意义(P<0.05)。结论  浮针或浮针联合其他疗法治疗颈椎病疗效更好、起效时间更短,但由于纳入研究的文献质量较低,发表文献存在一定偏倚,尚需大样本量、多中心、高质量的RCT进行验证。

Meta-analysis on Clinical Efficacy of Floating Needle in Treating Cervical Spondylosis

Abstract: Objective To evaluate clinical efficacy of floating needle in the treatment of cervical spondylosis (CS) based on meta-analysis, to provide references for clinical treatment. Methods Chinese Biomedical Literature Database (CBM), China Knowledge Resources Database (CNKI), Chinese Academic Journals Database (Wanfang Data), Chinese Sci-tech Periodicals Database (VIP), PubMed, Embase, and Cochrane Library databases were set as retrieval databases. Randomized controlled trial (RCT) about floating needle used to treat CS from the establishment to February 2020 of the above databases were retrieved and collected. Cochrane5.1.0 bias risk assessment tool was used to evaluate the methodological quality of the included literature and RevMan5.3 was used for Meta-analysis. Results Totally 15 articles were included, involving a sample size of 1284. The total effective rate (OR=2.88, 95% CI [1.92, 4.32], P<0.05), clinical recovery rate (OR=2.11, 95% CI [1.43, 3.13], P<0.05), immediate onset time (MD=-129.71, 95% CI [-138.38, -121.04],  P<0.05), overall VAS score (MD=-1.22, 95% CI [-2.28, -0.15], P<0.05); VAS score (nerve root type)   (MD=-1.79, 95% CI [-3.33, -0.25], P<0.05), VAS score (neck type) (MD=-0.41, 95% CI [-0.61, -0.20], P<0.05) of the experimental group were superior to the control group (P<0.05). Conclusion Floating needle and floating needle combined with other therapies have better efficacy and shorter onset time than other therapies for CS. However, due to the low quality of the literature included in the study and a certain bias in the published literature, large sample-sized, multi-center, and high-quality RCTs are still required for verification.

颈椎病(Cervical Spondylosis)又称颈椎综合征、颈椎骨关节病,是一种由颈椎间盘退行性改变及其继发病理改变累及周围组织结构而出现的现代常见疾病[1]。各地区的颈椎病流行病学调查结果存在差异,总体以40岁以上人群为主,伏案工作者发病率较高,青少年患病率呈逐年升高且年轻化趋势明显[2-4]

浮针是通过在患肌附近或邻近肢体皮下疏松结缔组织进行大幅度扫散,配合肌肉再灌注的一种物理疗法。患肌指在放松状态下,全部或部分仍处于紧张状态的肌肉[5]。……
