【摘要】 目的 探討现代根管治疗术联合米诺环素根管注入在牙周牙髓综合征患者中的临床效果。方法 60例牙周牙髓综合征患者， 按照随机数字表法分为对照组和观察组， 各30例。对照组给予现代根管治疗术联合牙周袋内局部注射盐酸米诺环素， 观察组给予现代根管治疗术联合米诺环素根管注入。比较两组临床疗效、牙周指数及不良反应发生情况。结果 观察组患者治疗总有效率93.33%显著高于对照组的73.33%， 差异具有统计学意义（P<0.05）。治疗后， 两组菌斑指数（PLI）、牙龈指数（GI）、牙周袋探诊深度（PD）均小于治疗前， 差异具有统计学意义（P<0.05）;且观察组患者PLI、GI、PD分别为（0.64±0.20）、（0.63±0.19）、（2.49±0.38）mm， 均明显小于对照组的（0.81±0.29）、（0.91±0.29）、（3.11±0.41）mm， 差异均有统计学意义（P<0.05）。观察组患者不良反应发生率为6.7%（2/30）， 对照组为10.0%（3/30）， 比较差异无统计学意义（P>0.05）。结论 现代根管治疗术联合米诺环素根管注入能显著提高牙周牙髓综合征患者的临床治疗效果， 且不增加不良反应。

【Abstract】 Objective   To discuss the clinical effect of modern root canal therapy combined with minocycline injection via root canal in patients with periodontal pulp syndrome. Methods   A total of 60 patients with periodontal pulp syndrome were divided into control group and observation group by random numerical table， with 30 cases in each group. The control group received modern root canal therapy combined with local injection of minocycline hydrochloride， and the observation group received  modern root canal therapy combined with minocycline injection via root canal. The clinical efficacy， periodontal index and occurrence of adverse reactions were compared between the two groups. Results   The total effective rate 93.33% of the observation group was significantly higher than that of the control group 73.33%， and the difference was statistically significant （P<0.05）. After treatment， the plaque index （PLI）， gingival index （GI）， probing depth （PD） of the two groups was less than those before treatment， and the difference was statisically significant （P<0.05）. After treatment， PLI， GI and PD of the  observation group were （0.64±0.20）， （0.63±0.19） and （2.49±0.38） mm， which were obviously less than those of the control group （0.81±0.29）， （0.91±0.29） and （3.11±0.41） mm， and the difference was statistically significant （P<0.05）. The incidence of adverse reactions of the observation group was 6.7%（2/30）， which was 10.0%（3/30） of the control group， and the difference was not statistically significant （P>0.05）. Conclusion   Modern root canal therapy combined with minocycline injection via root canal can significantly improve the clinical effect of patients with periodontal pulp syndrome without increasing adverse reactions.

牙周牙髓综合征表现为牙体牙髓病、牙周病的联合病理改变， 患者常伴有自发痛和冷热激发痛， 治疗时需对多个病灶都进行兼顾治疗[1]， 抗菌治疗， 确保彻底消除感染源， 进行对患者的牙周、牙髓综合治疗。米诺环素是临床常用的抗菌药物， 牙周袋内局部注射不能彻底帮助患者消除感染源， 为了提高手术的治疗效果， 往往采用根管注射米诺环素结合现代根管治疗术进行治疗， 本研究对比米诺环素局部注射与根管注入米诺环素的不同使用效果， 探讨米诺环素联合现代根管治疗术对牙周牙髓综合征患者的临床效果及应用价值， 现将研究结果报告如下。
