血清镁水平对维持血液透析患者内瘘血栓形成的影响初探

[摘要] 目的 探讨血清镁水平对维持血液透析患者内瘘血栓形成的影响。方法 方便选取2018年4月—2019年6月该院收治的30例内瘘血栓患者作为试验组,选取同时期30例非内瘘血栓患者作为对照组,分别对两组患者的血红蛋白、钙、磷、C反应蛋白(CRP)、血清白蛋白(ALB)、血浆纤维蛋白(Fib)、镁(Mg)等水平进行测量,观察两组患者镁水平与维持血液透析患者内瘘血栓形成发生的相关性。结果 试验组患者的血红蛋白、Ca、P、CRP、ALB、Fib、Mg含量分别为(172.63±28.95)g/L、(3.06±0.41)mmol/L、(1.86±0.74)mmol/L、(31.26±5.02)mg/L、(36.59±6.42)mg/L、(5.96±0.84)g/L、(1.01±0.12)mmol/L,明显高于对照组的(126.35±24.59)g/L、(2.28±0.35)mmol/L、(1.25±0.46)mmol/L、(25.83±4.73)mg/L、(28.76±5.32)mg/L、(5.43±0.75)g/L、(0.87±0.13)mmol/L,差异有统计学意义(t=6.674、7.925、3.835、4.312、5.144、2.578、4.334,P=0.000、0.000、0.000、0.000、0.000、0.013、0.000);经COX回归性分析后可知,镁、血红蛋白、钙、磷、ALB、CRP、Fib与维持血液透析患者内瘘血栓形成发生具有较大的相关性,差异有统计学意义(P<0.05)。结论 血清镁水平对维持血液透析患者内瘘血栓形成具有较大的影响,即水平越低,形成率越大。

[Abstract] Objective To investigate the effect of serum magnesium level on fistula and thrombosis in maintenance hemodialysis patients. Methods Thirty patients with internal fistula and thrombosis treated in the hospital from April 2018 to June 2019 were convenient scelection selected as the test group, and 30 patients with non-fistula and thrombosis during the same period were selected as the control group. The hemoglobin, calcium, the levels of phosphorus, C-reactive protein (CRP), serum albumin (ALB), plasma fibrin (Fib),magnesiom(Mg) were measured, and the correlation between magnesium levels in patients in both groups and the occurrence of fistula thrombosis in hemodialysis patients was observed. Results The hemoglobin, Ca, P, CRP, ALB, Fib,Mg contents in the test group were (172.63±28.95) g/L, (3.06±0.41) mmol/L, (1.86±0.74) mmol/L, and (31.26±5.02) mg/L, (36.59±6.42) mg/L, (5.96±0.84) g/L, (1.01±0.12)mmol/L significantly higher than (126.35±24.59) g/L, (2.28±0.35) mmol/L, (1.25±0.46) mmol/L, (25.83±4.73) mg/L, (28.76±5.32) mg/L, (5.43±0.75) g/L, (0.87±0.13)mmol/L, compared with significant differences (t=6.674, 7.925, 3.835, 4.312, 5.144, 2.578,4.334,P=0.000, 0.000, 0.000, 0.000, 0.000, 0.013,0.000); COX regression analysis shows that magnesium, hemoglobin, calcium, phosphorus, ALB, CRP, Fib and maintenance of internal fistula thrombosis in hemodialysis patients have Large correlation, and magnesium is closely related (P<0.05). Conclusion Serum magnesium level has a greater impact on fistula thrombosis in maintenance hemodialysis patients, that is, the lower the level, the greater the formation rate.

對于维持血液透析患者而言,其多存在脂质代谢紊乱的不良情况,若未予以及时有效的治疗,则将导致其发生内瘘血栓形成的不良事件,此事件将会在较大程度上增加患者的死亡率[1-2]。依据相关的临床研究显示[3],作为维持正常生理功能所必需的元素,镁参与机体内多种酶的合成或激活活动,并且对机体代谢、神经、肌肉与心血管功能起到良好的调节作用。但目前临床中关于镁影响脂代谢的机制尚未具有统一定论[4],该文将探讨2018年4月—2019年6月该院收治的30例内瘘血栓患者血清镁水平对维持血液透析患者内瘘血栓形成的影响。报道如下。
