【摘 要】目的：探討居家护理对社区老年高血压患者的管理效果研究。方法：选取近一年内北京市丰台区南苑社区卫生服务中心管辖的老年高血压患者200例，对患者进行居家护理，比较居家护理前后患者血压控制情况。结果：通过对老年高血压患者为期12个月的家居护理，护理后患者血压改善情况明显优于护理前，两组数据比较具有统计学意义（t=17.732，8.679，P<0.05），居家护理前服药依从率56.00%明显低于居家护理后服药依从率87.00%，数据比较具有统计学意义（χ2=47.160，P<0.05）。结论：对老年高血压患者进行居家护理，可以明显使患者的血压得到控制，用药依从性明显提高。

Effect of Home Nursing on Management of Elderly Hypertensive Patients in Community

【Abstract】Objective：To explore the Management Effect of Home Nursing on Elderly Hypertensive Patients in Community.Methods：200 elderly patients with hypertension under the jurisdiction of Nanyuan Community Health Service Center in Fengtai District of Beijing in the past year were selected. Home care for patients.Compare patients' blood pressure control before and after home care.Results：The 12-month home care of elderly hypertensive patients improved the blood pressure of the patients after the care was significantly better than before the care.The two groups of data were statistically significant （t =17.732，8.679，P<0.05）.The compliance rate of medication before home care was 56.00%，which was significantly lower than the compliance rate of medication after home care was 87.00%，and the data was statistically significant （χ2=47.160，P<0.05）.Conclusion：Home care for elderly hypertensive patients can significantly control patients' blood pressure and significantly improve medication compliance.

随着我国人口老龄化趋势的加重，高血压的发病率以及检出率逐年升高，高血压疾病已经演变成威胁人体健康和降低生活质量的重要疾病，患者承受 痛苦的同时，也给社会及家庭增加了压力[1]。在对高血压患者进行有效治疗的同时有效的护理工作也具有重要的作用，传统的护理模式仅给予药物、饮食调整等有一定局限性。本研究重点探讨居家护理对社区老年高血压患者管理效果的影响，结果如下

1 资料与方法

1.1一般资料：选取近一年内北京市丰台区南苑社区卫生服务中心管辖的老年高血压患者200例，对入选患者进行居家护理，其中男122例，女78例。年龄61～87岁。

1.2诊断标准：根据《中国高血压基层管理指南》（2014年修订版）[2] ，高血压定义为：在未使用降压药物的情况下，收缩压≥140mmHg和/或舒张压≥90mmHg。

1.3居家护理方法：社区医护人员对患者的社会家庭环境、心理状态、高血压病相关知识的掌握情况、生存质量、营养情……
