【摘要】 目的：探讨急性胃十二指肠溃疡穿孔的危险因素。方法：回顾性分析2013年1月-2015年12月笔者所在医院收治的285例急性胃十二指肠溃疡穿孔患者为观察组，选取笔者所在医院同期诊治的285例胃十二指肠溃疡患者为对照组。资料分析采用单因素及多因素Logistic分析。结果：急性胃十二指肠溃疡穿孔多发生于冬春季节，以十二指肠为多见。单因素分析显示，患者性别、年龄、身体质量指数（BMI）、职业、文化程度、服用NSAIDs药物是急性胃十二指肠溃疡穿孔的危险因素（P<0.05）。多因素分析显示，患者的年龄、BMI、职业、文化程度、服用NSAIDs药物是急性胃十二指肠溃疡穿孔的独立危险因素（P<0.05）。结论：对于男性、年龄≥60岁、长期服用NSAIDs药物、文化程度及BMI指数低，并且职业为农民或无业患者，冬春季节应格外注意病情的发作，警惕穿孔的发生。

【Abstract】 Objective：To explore the risk factors of acute perforation of gastroduodenal ulcer.Method：285 cases of acute gastroduodenal ulcer perforation in our hospital from January 2013 to December 2015 were selected as observation group.285 patients with gastroduodenal ulcer were selected as control group.Data were analyzed by single factor and multifactor Logistic review.Result：Acute gastroduodenal ulcer perforation often occurs in winter and spring，with the duodenum as the most common.The single factor analysis showed that gender，age，body mass index（BMI），occupation，education level and NSAIDs might be risk factors for acute gastroduodenal ulcer perforation（P<0.05）.Multi-factors analysis showed that age，BMI，occupation，education level and NSAIDs were independent risk factors for acute gastroduodenal ulcer perforation（P<0.05）.Conclusion：For men，over 60 years of age，taking NSAIDs for a long time，with low education level and BMI index，and occupational status as peasants or unemployed patients，winter and spring season should pay special attention to the occurrence of gastroduodenal ulcer，be alert to the occurrence of perforation.

急性胃十二指腸溃疡穿孔是消化性溃疡的严重并发症，也是常见的外科急腹症之一。其起病急、病情变化快，如若诊治不当可危及生命。近来溃疡穿孔的发生率呈上升趋势，发病年龄渐趋高龄化[1]。如何有效预防急性胃十二指肠溃疡穿孔是临床关注的重要问题。笔者对2013年1月-2015年12月笔者所在医院诊疗的胃十二指肠溃疡患者的临床资料进行回顾性分析，对引起胃十二指肠溃疡穿孔的危险因素进行病例对照研究，并报告如下。

1 资料与方法

1.1 一般资料

回顾性分析2013年1月-2015年12月笔者所在医院收治的285例急性胃十二指肠溃疡穿孔患者作为观察组，选取笔者所在医院同期诊治的285例胃十二指肠溃疡患者作为对照组。年龄14～93岁。配对选择条件为：（1）民族构成相同;（2）长期居住地为客家地区，生活习俗、经济水平等相似。……
