摘 要 目的：探讨临床药师有效的参与并干预骨科植入物手术围手术期预防使用抗菌药物的作用，以促进合理用药。方法：收集并分析2012年（干预前）、2013年（干预1年）、2014年（干预2年）和2016年（干预4年）的骨科植入物手术病历，调查比较临床药师干预前后骨科植入物手术围手术期使用抗菌药物的情况，分析其用药合理性。结果：临床药师干预骨科植入物围手术期预防使用抗菌药物后，在药物品种选择、预防用药时间等方面较干预前规范。结论：临床药师对骨科植入物手术围手术期预防使用抗菌药物干预有效，但仍存在不合理使用情况，有待继续改进。

Analysis of the intervening effect of preventive use of antibacterials during the perioperative period in orthopedic implants surgery

ABSTRACT Objective： To discuss clinical pharmacists effectively participated in and interfered in the preventive and rational use of antibacterials during the perioperative period in orthopedic implants surgery， so as to promote rational drug use. Methods： Medical records underwent orthopedic implant surgery in 2012， 2013， 2014， 2016 were collected and their rational use of antibiotics during the perioperative period was analyzed. Results： After clinical pharmacists intervention on preventive use of antibacterials， the use of antibacterials was more rational than before in such aspects as drug choice， duration of drug use and so on. Conclusion： Clinical pharmacists intervention on preventive use of antibacterials during the perioperative period in orthopedic implants surgery was proved to be effective， but there are still some unreasonable use cases， which need to be further improved.

作為20世纪医药卫生最重要的成就之一的抗菌药物的诞生为人类带来了莫大的福音，尤其在外科手术中的应用保证了病人的安全，减少了术后感染，提高了治愈率；在预防和治疗感染性疾病中也起着至关重要的作用。但在临床广泛应用的同时，也普遍存在不合理应用的现象。现今，抗菌药物的滥用已成为一项全球性公共卫生问题。近年来，卫生部出台了一系列文件或指导原则[1-4]，以规范抗菌药物的合理使用，也反复强调了围手术期合理使用抗菌药物的重要性。围手术期合理预防使用抗菌药物也已成为临床药学工作的重点内容。本文以我院临床药师干预骨科植入物手术围手术期预防使用抗菌药物为切入点，分析和探讨干预效果及存在问题。……
