摘 要 在精准医学时代，手术仍是乳腺癌多学科治疗的关键。随着早期诊断水平的提高、抗肿瘤药物的发展、手术理念的更新以及人们对美好生活的追求，乳腺癌外科治疗理念已发生重大变化，正转向如何在肿瘤根治与功能性和美学之间寻求平衡，优化治疗策略，以实现“最小的创伤、最佳的功能、最美的形态”的目标。

Oncoplastic breast-conserving options for breast cancer： radical mastectomy or surgical aesthetics

ABSTRACT Under a new era of precision medicine， surgery is still of vital importance in multidisciplinary treatment of breast cancer. With the improvement of early diagnostic imaging and detection levels， anti-tumor drugs effect， operation concept progress and peoples pursuit of a better life， the concept of surgical treatment for breast cancer has undergone drastic changes， seeking a new balance in radical treatment and functional aesthetics and optimizing treatment strategy so as to achieve “the smallest trauma， the optimum function and the most perfect appearance”.

乳腺癌是女性的最常见恶性肿瘤类型，2012年全球新确诊的女性乳腺癌患者数达约170万人[1-3]。尽管乳腺癌发病率逐年升高，但因其早期诊出率提高，加之微创外科、适形放疗、化疗、靶向治疗和内分泌治疗等技术或药物得到快速发展，乳腺癌患者的长期生存率反得到了极大改善。2008年，世界卫生组织将乳腺癌重新定义为慢性病。乳房作为女性的第二性征，切除后会给女性的身心和家庭带来巨大的创伤和社会学问题。乳腺癌的治疗理念历经百年变革，已由最初的“一刀切”发展为精准医学理念指导下的个体化治疗，从人文、美学和患者的心理健康以及家庭社会学角度出发，在力求实现治疗目标的同时以“最小的创伤”达到“最美的形态和最佳的功能”正成为乳腺肿瘤外科的发展趋势。

1 乳腺癌手术的发展简史

历史上早期的乳腺癌手术野蛮、原始，钳夹、烧灼、腐蚀等诸多残酷的术式均有记载。直至19世纪中叶，随着麻醉、消毒、输血技术的发展和抗生素的出现，这才极大地推动了乳腺癌外科治疗变革。……
