[摘要] 目的 探討当归贝母苦参丸加味对湿热下注型妊娠小便淋痛的治疗效果。 方法 选择我院符合诊断标准的60例湿热下注型妊娠小便淋痛患者随机分为观察组30例和对照组30例，观察组采用中药当归贝母苦参丸加味口服治疗；对照组采用清淋颗粒口服治疗。对两组治疗前后临床症状、白细胞及红细胞转阴率进行比较，比较两组治疗效果。 结果 观察组治愈率高于对照组。治疗5 d、7 d后，治疗组红细胞转阴率及白细胞转阴率均高于对照组（P<0.05）；治疗3 d后，观察组尿频、尿痛临床症状好转率、消失率均高于对照组（P<0.05），治疗5 d、7 d后，观察组尿频、尿痛临床症状仅消失率高于对照组（P<0.05）。 结论 当归贝母苦参丸加味治疗湿热下注型妊娠小便淋痛疗效确切，值得临床应用。

Observation on the curative effect of modified Danggui Beimu Kushen Pills in the treatment of 30 cases of pregnancy strangury with the type of downward flow of damp-heat

[Abstract] Objective To explore the curative effect of modified Danggui Beimu Kushen pills in the treatment of pregnancy strangury with the type of downward flow of damp-heat. Methods 60 patients with pregnancy strangury with the type of downward flow of damp-heat who conformed to the diagnosis criteria in our hospital were selected and randomly divided into the observation group（n=30） and the control group （n=30）. The observation group was orally given modified Danggui Beimu Kushen pills， and the control group was orally given Qinglin granules. The clinical symptoms， and negative conversion ratio of white blood cells and red blood cells were compared before and after treatment in both groups. The curative effect was compared between the two groups. Results The cure rate in the observation group was higher than that in the control group. After the treatment for 5 days and 7 days， the negative conversion rates of red blood cells and white blood cells were higher in the treatment group than in the control group（P<0.05）. After the treatment for 3 days， the improvement rate and disappearance rate of clinical symptoms such as frequent urination and odynuria in the observation group were higher than those in the control group（P<0.05）. After the treatment for 5 days and 7 days， only the disappearance rate of clinical symptoms such as frequent urination and odynuria in the observation group was significantly higher than that in the control group（P<0.05）. Conclusion Modified Danggui Beimu Kushen pills in the treatment of pregnancy strangury with the type of downward flow of damp-heat has an exact curative effect， which is worthy of clinical application.

妊娠期间出现尿频、尿急、淋沥涩痛等，称“妊娠小便淋痛”，亦称“子淋”，类似于西医的妊娠合并泌尿系感染[1]。子淋是常见的妊娠并发症，严重者可出现寒战、高热、体温升高可达39℃～40℃，甚至可由高热引起流产、早产[2]，甚至可诱发急性肾功能衰竭，发病率约占孕妇的7%[3]。西医一般主要以抗生素治疗，但孕妇由于常考虑胎儿抗拒使用抗生素或因过敏耐药等问题不能使用抗生素治疗，中药因其毒副作用较小且疗效确切，易被患者接受。

马大正教授善用经方治疗妇科疾病，药简效佳，许多经方中非妇科方剂马老师可用来治疗妇科疾病。本文就马老师应运经方“当归贝母苦参丸加味”治疗湿热下注型妊娠小便淋痛一病进行临床治疗，现报道如下。
