【摘要】 目的：為了检测“双C”方案治疗2型糖尿病并发乳腺癌的临床效果。方法：按照纳入和排除标准选取2011年10月-2014年2月来笔者所在医院就诊的2型糖尿病并发乳腺癌的患者56例进行回顾性研究。其中28例患者为“双C”方案治疗组，另外28例则采用普通注射胰岛素的治疗方法。对两组患者治疗前后的血糖水平进行检测，比较两组患者治疗前后的血糖控制情况，以及血糖波动的幅度，比较两组患者治疗前后24 h内胰岛素使用量、住院时间及患者对治疗效果的满意程度。结果：治疗前两组患者胰岛素使用量比较差异无统计学意义（P>0.05），治疗后两组患者胰岛素使用量比较差异有统计学意义（P<0.05）。手术前两组患者血糖水平和血糖波动幅度比较差异无统计学意义（P>0.05），两组患者手术前后血糖水平和血糖波动幅度比较差异均有统计学意义（P<0.05）。治疗前两组患者糖化血红蛋白水平比较差异无统计学意义（P=1.43），治疗后双C组患者HbA1c水平明显低于对照组，差异有统计学意义（P=0.03）。双C组患者住院时间和满意度均优于对照组患者，差异均有统计学意义（P<0.05）。结论：使用“双C”方案对2型糖尿病并发乳腺癌具有良好的临床治疗效果，并且要优于普通治疗方法。

【Abstract】 Objective：To test the "double C" in the treatment of type 2 diabetes mellitus in breast cancer clinical results.Method：According to inclusion and exclusion criteria，56 cases of type 2 diabetes mellitus in patients with breast cancer to the authors hospital from October 2011 to February 2014were studied retrospectively.28 patients were “double C” in the treatment group，while 28 cases were common treatments using insulin injections.The blood glucose levels in the two groups of patients before and after testing，blood glucose control before and after comparison of two groups of patients，as well as the magnitude of fluctuations in blood sugar，24 h in the amount of insulin using two groups were compared before and after treatment，hospitalization and patient satisfaction with the treatment effect degree.Result：There was no statistically significant difference in insulin use between the two groups（P>0.05），while there was statistically significant difference in insulin use between the two groups after treatment（P<0.05）.There was no statistically significant difference between the blood glucose levels and the blood glucose fluctuation in the two groups before the operation（P>0.05），the differences in blood glucose level and blood glucose fluctuation were statistically significant between the two groups（P<0.05）.There was no significant difference in glycosylated hemoglobin levels in the two groups before treatment（P=1.43），after treatment the level of HbA1c was significantly lower than that in the control group，the difference was significant（P=0.03）.The Length of stay and patient satisfaction in double C group were better than the control group，the differences were statistically significant （P<0.05）.Conclusion：“ Double C” program for type 2 diabetes mellitus with breast cancer have good clinical results， and superior to ordinary treatment.endprint

联合使用胰岛素泵进行持续的皮下输注（continuous subcutaneous insulin infusin，CSII）和对患者进行动态血糖的监测系统（continuous glucose monitoring system，CGMS）简称“双C”治疗。这种方法根据血糖的动态监测结果，对胰岛素泵的剂量及时进行调整，从而更容易使患者血糖出现明显的降低，并很容易控制在标准之内。CSII不但可以模拟人体内胰腺分泌胰岛素，而且可以将血糖和糖化血红蛋白在24 h内连续控制在正常范围内，从而使体内胰岛素的水平处于正常的生理浓度，这种治疗手段是一种对糖尿病患者进行强化治疗的先进的技术方法[1]。
