[摘要] 目的 探究不同时期胰岛素治疗妊娠合并糖尿病孕妇对妊娠结局的影响。 方法 以2017年1月—2018年1月该院治疗的妊娠合并糖尿病孕妇90例纳入研究，分为对照组和观察组，各45例，对照组妊娠32周应用胰岛素，观察组在32周前应用胰岛素，对比两组治疗效果。 结果 经治疗，两组空腹血糖及餐后2 h血糖显著降低，观察组低于对照组;观察组发生妊娠期高血压、死胎、产后出血及感染的比率低于对照组，发生剖宫产、早产的比率高于对照组。结论 妊娠合并糖尿病孕妇妊娠时间不超过32周时进行胰岛素治疗，可有效改善妊娠结局，值得采纳应用。

Effect of Insulin Treatment on Pregnancy Outcome in Pregnant Women with Diabetes at Different Stages

[Abstract] Objective To investigate the effect of insulin treatment on pregnancy outcome in pregnant women with diabetes at different stages. Methods A total of 90 pregnant women with diabetes mellitus who were treated in the hospital from January 2017 to January 2018 were enrolled in the study. They were divided into 45 groups in the control group and the observation group respectively. The control group used insulin for 32 weeks and the observation group applied that before 32 weeks， comparing the effects of the two groups. Results After treatment， fasting blood glucose and 2 h postprandial blood glucose were significantly lower in the two groups. The observation group was lower than the control group. The incidence of gestational hypertension， stillbirth， postpartum hemorrhage and infection in the observation group was lower than that of the control group， and cesarean section and premature delivery occurrance ratio was higher than the control group. Conclusion Insulin therapy for pregnant women with diabetes mellitus without gestation for more than 32 weeks can effectively improve pregnancy outcomes and was worthy of application.

妊娠合并糖尿病是一種妇产科常见疾病，根据其发生类型可分为孕前糖尿病和妊娠期糖尿病两种症型，前者为孕妇在妊娠前即患有糖尿病，已确诊，或妊娠期首次诊断出来;后者是指在妊娠期发生的糖代谢异常疾病，常并发高血压与酮症酸中毒，是羊水过多、早产等不良妊娠结局的高危因素[1-2]。为了保障母婴安全，一般而言妊娠合并糖尿病孕妇都会应用胰岛素疗法，以便将血糖控制在合理范围内。该次试验旨在探究不同时期胰岛素治疗妊娠合并糖尿病孕妇对妊娠结局的影响，分析2017年1月—2018年1月间在该院治疗的妊娠合并糖尿病孕妇90例的临床资料，现报道如下。
