对比增强核磁共振技术对颈动脉易损斑块的评估价值

摘 要 目的:探讨对比增强核磁共振成像技术(contrast-enhanced magnetic resonance imaging,CEMRI)对颈动脉易损斑块的评估价值。方法:对普通二维多普勒超声检查发现颈动脉斑块并且血管狭窄程度>50%者,进一步行CEMRI检查,记录并分析检查结果。结果:共计76名患者进行了CEMRI检查(其中男性53例,女性23例),59例(77.6%)患者发现易损斑块,总易损斑块数为135个。其中脂质坏死核59个,有钙化斑块46个,斑块内出血30个。结论:CEMRI技术可以显示颈动脉斑块结构特点及成分特征,客观评价斑块的稳定性。

An value of contrast-enhanced magnetic resonance imaging in the assessment of vulnerable plaques*

ABSTRACT Objective: To discuss the value of contrast-enhanced magnetic resonance imaging (CEMRI) in the assessment of carotid vulnerable plaques. Methods: Patients whose stenosis of carotid artery is more than 50% were further examined by CEMRI and the results were recorded and analyzed. Results: There were 76 patients who took the examination of CEMRI (male 53, female 23), and 135 cases of carotid vulnerable plaques were found in 59 (77.6%) patients, in which there were 59 cases of lipid-rich necrosis core, 46 cases of calcified plaque and 30 cases of intraplaque bleeding. Conclusion: CEMRI can be used to show the characteristics of structure and composition of the carotid plaques and assess the stability of the carotid plaques.

动脉粥样硬化斑块的形成是导致脑卒中的重要危险因素,易损斑块的破裂或脱落是发生临床事件的主要原因[1-2]。对于颈动脉易损斑块评估主要分为有创性检查與无创性检查。有创性检查包括血管内超声、颈动脉内膜剥脱术后病理检查等。无创性检查包括斑块超声、磁共振(magnetic resonance imaging, MRI)、计算机断层扫描血管造影技术(computed tomography angiography, CTA)等,其中对比增强磁共振(contrast-enhanced magnetic resonance imaging, CEMRI)技术对斑块内结构显示清晰、有良好的病理相关性,成为目前斑块稳定性分析的热门方法[3]。我们采用CEMRI技术,对颈动脉斑块的信号特点,如斑块内部钙化、脂质核心、斑块内出血等进行分析,评估斑块的稳定性。

1 资料与方法

1.1 研究对象

选取2015年1月—2016年12月在上海市第六人民医院和上海市第八人民医院因心脑血管病就诊和住院的76名患者纳入研究。由专人采集记录患者的基数资料:年龄、性别、吸烟史、饮酒史、既往史和血液生化指标等。本……
