[摘要] 目的 了解本院住院患者合理用药情况，为临床规范合理用药提供参考。 方法 随机抽查2015年1～12月本院住院患者出院病历每月150份，共计1800份，审查医嘱用药的合理性，登记不合理用药情况，进行统计分析。 结果 其中96份病历发现110处用药不合理，不合理用药主要表现为：用药指征不明确 （20.91%）、用法用量不适宜（20.00%）、药物遴选不当（17.27%）、药物不良相互作用（12.73%）、溶媒选择不适宜（9.09%）、禁忌症用药（6.36%）等。 结论 本院住院患者用药仍存在一些不合理用药现象，有待加强临床用药管理，促进临床合理用药。

[Abstract] Objective To understand the rational drug use in hospitalized patients of our hospital，and to provide reference for rational use of drugs in clinical practice. Methods Discharge records of inpatients in our hospital from January 2015 to December 2015 were randomly sampled with 150 copies a month，a total of 1800 copies.Rational use of prescription medication，irrational use of drugs were examined and statistically analyzed. Results 96 of these cases found that the use of 110 drugs unreasonable.Irrational drug use were mainly manifested as：indication of medication is not clear（20.91%），dosage is not suitable（20%），improper drug selection（17.27%），adverse drug interactions（12.73%），choice of solvent not suitable （9.09%），contraindications （6.36%），etc. Conclusion There are still some irrational use of drugs in the inpatients of our hospital，which need to strengthen the management of clinical medication and promote the rational use of drugs in clinic.

《處方管理办法》[1]规定，医师开具处方和药师调剂处方应当遵循安全、有效、经济的原则。医疗机构应当建立处方点评制度，对处方实施动态监测及超常预警，登记并通报不合理处方，对不合理用药予以干预。根据相关文件精神，本研究随机抽取2015年1～12月本院住院患者出院病历1800份，审查医嘱用药合理性，旨在为临床规范合理用药提供参考。

1 资料与方法

1.1 一般资料

随机抽取2015年1～12月本院住院患者出院病历每月150份，共计1800份。

1.2 方法

逐份审查每份出院病历医嘱的用药合理性，按照已设计好的表格，登记不合理用药医嘱具体情况，包括病历号、患者性别、年龄、临床诊断，药品剂型、剂量、用药途径、疗程等，结合患者病史、疾病情况、辅助检查和临床化验等对治疗用药进行评价，不合理用药情况用Excel软件统计分析。……
